摘要:
目的:探討體外沖擊波療法聯(lián)合自體骨髓血活性濃聚物和富血小板血漿局部注射治療下肢骨折不愈合的臨床療效。方法:2019年3月至2023年6月,采用體外沖擊波療法聯(lián)合自體骨髓血活性濃聚物和富血小板血漿局部注射治療下肢骨折不愈合患者33例。男24例,女9例。年齡17~62歲,中位數(shù)43歲。肥大型骨折不愈合23例,萎縮型骨折不愈合10例。所有患者均接受過內固定術治療,均為非感染性骨折不愈合,骨折端間隙均小于5 mm,骨折端對位對線均良好,內固定均無明顯松動。體外沖擊波治療共4次,每次治療間隔7 d; 在體外沖擊波治療后第2天,于骨折端注射自體骨髓血活性濃集物和富血小板血漿,連續(xù)治療4次,每次治療間隔7 d。治療后隨訪觀察骨折端愈合情況。結果:33例患者均獲隨訪,隨訪時間3~6個月,中位數(shù)4.3個月。23例肥大型骨折不愈合患者經(jīng)治療后,骨折端均獲骨性愈合; 10例萎縮型骨折不愈合患者經(jīng)治療后,6例骨折端獲骨性愈合、4例骨折端未愈合。10例患者在治療結束后3個月骨折端獲骨性愈合,19例患者在治療結束后6個月骨折端獲骨性愈合; 4例患者在治療結束后6個月骨折端未愈合,均為萎縮型骨折不愈合患者,后期行內固定聯(lián)合植骨術后6個月骨折端獲骨性愈合。結論:采用體外沖擊波療法聯(lián)合自體骨髓血活性濃集物和富血小板血漿局部注射治療下肢骨折不愈合,能夠明顯促進骨折愈合。
