摘要:
目的:觀察關節(jié)鏡下帶骨膜髂骨移植聯(lián)合富血小板血漿(platelet-rich plasma,PRP)凝膠注射治療HeppleV型距骨骨軟骨損傷(osteochondral lesion of the talus,OLT)的臨床療效和安全性。方法:2019年1月至2022年8月,采用關節(jié)鏡下帶骨膜髂骨移植聯(lián)合PRP凝膠注射治療HeppleV型OLT患者24例。男14例,女10例。年齡16~55歲,中位數(shù)32歲。病程6~18個月,中位數(shù)14個月。測量手術前后的踝關節(jié)活動度。采用疼痛視覺模擬量表(visual analogue scale,VAS)評分評價足部疼痛情況,采用美國足與踝關節(jié)協(xié)會(American Orthopedic Foot and Ankle Society,AOFAS)踝與后足評分標準評價踝關節(jié)功能。術后隨訪觀察并發(fā)癥發(fā)生情況。結(jié)果:所有患者均獲隨訪,隨訪時間12~30個月,中位數(shù)20個月。術后1例患者出現(xiàn)小腿肌間靜脈血栓,經(jīng)對癥治療后好轉(zhuǎn)。所有患者均未出現(xiàn)神經(jīng)損傷和切口感染。踝關節(jié)活動度,術前42.16°±5.88°、術后1個月52.28°±7.24°、術后3個月58.44°±8.46°、末次隨訪時62.36°±10.24°。足部疼痛VAS評分,術前(5.88±1.48)分、術后1個月(3.64±1.20)分、術后3個月(2.12±0.88)分、末次隨訪時(1.24±0.68)分。AOFAS踝與后足評分,術前(52.22±9.46)分、術后1個月(78.28±7.24)分、術后3個月(84.48±6.68)分、末次隨訪時(90.44±6.44)分。結(jié)論:關節(jié)鏡下帶骨膜髂骨移植聯(lián)合PRP凝膠注射治療HeppleV型OLT,可以增加踝關節(jié)活動范圍、減輕足部疼痛、促進踝關節(jié)功能恢復,且安全性較高。
