摘要:
目的:系統(tǒng)評(píng)價(jià)手法復(fù)位聯(lián)合經(jīng)皮椎體后凸成形術(shù)(percutaneous kyphoplasty,PKP)治療骨質(zhì)疏松性椎體壓縮骨折(osteoporotic vertebral compression fracture,OVCF)的療效與安全性。方法:應(yīng)用計(jì)算機(jī)檢索中國(guó)知網(wǎng)、維普網(wǎng)、萬方數(shù)據(jù)庫(kù)、PubMed及Cochrane Library中關(guān)于手法復(fù)位聯(lián)合PKP治療OVCF的臨床對(duì)照試驗(yàn)研究文獻(xiàn)(觀察組干預(yù)措施為手法復(fù)位聯(lián)合PKP,對(duì)照組干預(yù)措施為單純PKP)。檢索時(shí)限均為建庫(kù)至2023年10月1日。根據(jù)文獻(xiàn)納入和排除標(biāo)準(zhǔn)對(duì)檢索到的文獻(xiàn)進(jìn)行篩選后,對(duì)納入文獻(xiàn)進(jìn)行質(zhì)量評(píng)價(jià),提取文獻(xiàn)中的數(shù)據(jù)進(jìn)行Meta分析。結(jié)果:共檢索出文獻(xiàn)233篇,經(jīng)篩選后最終納入文獻(xiàn)9篇,均為中文文獻(xiàn)。Meta分析結(jié)果顯示,觀察組疼痛視覺模擬量表評(píng)分低于對(duì)照組[MD=-0.67,95%CI(-1.15,-0.20),P=0.006],傷椎后凸Cobb角改善和椎體前緣高度的恢復(fù)均優(yōu)于對(duì)照組[MD=-1.54,95%CI(-2.67,-0.41),P=0.007; MD=2.81,95%CI(2.02,3.59),P=0.000],但2組在Oswestry功能障礙指數(shù)和骨水泥滲漏率方面的差異均無統(tǒng)計(jì)學(xué)意義[MD=-3.99,95%CI(-11.24,3.26),P=0.280; OR=0.48,95%CI(0.21,1.11),P=0.080]。結(jié)論:手法復(fù)位聯(lián)合PKP治療OVCF可有效緩解患者疼痛、改善傷椎后凸畸形、恢復(fù)傷椎前緣高度,療效優(yōu)于單純采用PKP治療,但二者在患者日常活動(dòng)能力改善及安全性方面相當(dāng)。
Abstract:
Objective:To systematically review the clinical outcome and safety of manipulative reduction combined with percutaneous kyphoplasty(PKP)for treatment of osteoporotic vertebral compression fracture(OVCF).Methods:All the clinical controlled trial articles about manipulative reduction combined with PKP for treatment of OVCF included from database's inception to October 1,2023 were retrieved from the China National Knowledge Infrastructure,Vip Database,Wanfang Database,PubMed and Cochrane Library through computer.The patients in observation group were treated with manipulative reduction and PKP,while the ones in control group with PKP alone.The articles were screened according to the inclusion and exclusion criteria.The methodological quality of researches in the included articles was evaluated,and then a Meta-analysis was conducted.Results:Two hundred and thirty-three articles were searched out.After screening,9 Chinese articles were included in the final analysis.The results of Meta-analysis revealed that the pain visual analogue scale(VAS)score was lower,the improvement of kyphotic Cobb angle of injured vertebrae and restoration of anterior border height of vertebral body were better in observation group compared to control group(MD=-0.67,95%CI(-1.15,-0.20),P=0.006; MD=-1.54,95%CI(-2.67,-0.41),P=0.007; MD=2.81,95%CI(2.02,3.59),P=0.000); while,the comparison of Oswestry disability index(ODI)and bone cement leakage incidence rate between the 2 groups revealed no significant differences(MD=-3.99,95%CI(-11.24,3.26),P=0.280; OR=0.48,95%CI(0.21,1.11),P=0.080).Conclusion:Manipulative reduction combined with PKP can effectively relieve pain,improve the kyphosis of injured vertebrae,and restore the anterior border height of injured vertebrae in treatment of OVCF,and its curative outcome is better than that of PKP alone; while,both are similar to each other in the improvement of activities of daily living and safety.
