摘要:
目的:探討圍手術(shù)期外周靜脈血中長鏈非編碼RNA(long non-coding RNA,lncRNA)PAX8-AS1及微RNA(microRNA,miRNA)-1252-5p表達(dá)水平對脛骨骨折內(nèi)固定術(shù)后骨折愈合的影響。方法:以2018年1月至2020年12月在宜興市人民醫(yī)院接受同一組醫(yī)生完成的鋼板內(nèi)固定或髓內(nèi)釘內(nèi)固定手術(shù)治療的脛骨骨折患者為研究對象。收集患者的性別、年齡、骨折類型、損傷類型、手術(shù)方式、吸煙史、飲酒史、高血壓史、糖尿病史等一般資料,隨訪觀察骨折愈合情況。采用熒光定量PCR檢測術(shù)前及術(shù)后1周、2周、4周、8周時患者外周靜脈血中l(wèi)ncRNA PAX8-AS1、miRNA-1252-5p的表達(dá)水平。根據(jù)術(shù)后4個月時的骨折愈合情況,將納入的病例分為正常愈合組及延遲愈合組。比較2組患者的一般資料及手術(shù)前后外周靜脈血中l(wèi)ncRNA PAX8-AS1、miRNA-1252-5p表達(dá)水平,分析影響骨折愈合的因素,評價外周靜脈血中l(wèi)ncRNA PAX8-AS1、miRNA-1252-5p表達(dá)水平對骨折延遲愈合的預(yù)測價值。結(jié)果:①一般結(jié)果。共納入250例患者,正常愈合組210例、延遲愈合組40例。延遲愈合組合并糖尿病比例、開放性骨折比例、高能量損傷比例均高于正常愈合組(χ2=28.245,P=0.000; χ2=24.881,P=0.000; χ2=34.469,P=0.000)。②外周靜脈血中l(wèi)ncRNA PAX8-AS1、miRNA-1252-5p表達(dá)水平檢測結(jié)果。術(shù)后2周和術(shù)后4周時,延遲愈合組外周靜脈血中l(wèi)ncRNA PAX8-AS1表達(dá)水平均高于正常愈合組(t=4.742,P=0.000; t=2.753,P=0.006),miRNA-1252-5p表達(dá)水平均低于正常愈合組(t=4.079,P=0.000; t=2.538,P=0.012)。③脛骨骨折內(nèi)固定術(shù)后骨折愈合影響因素分析結(jié)果。廣義估計方程分析結(jié)果顯示,開放性骨折、高能量損傷及術(shù)后2周時外周靜脈血中l(wèi)ncRNA PAX8-AS1表達(dá)水平升高和miRNA-1252-5p表達(dá)水平降低是脛骨骨折內(nèi)固定術(shù)后骨折愈合的影響因素(β=29.382,Wald χ2=8.589,P=0.000; β=22.771,Wald χ2=9.766,P=0.000; β=57.418,Wald χ2=13.735,P=0.000; β=-81.524,Wald χ2=2.392,P=0.000)。④外周靜脈血中l(wèi)ncRNA PAX8-AS1、miRNA-1252-5p表達(dá)水平對脛骨骨折內(nèi)固定術(shù)后骨折延遲愈合的預(yù)測價值評價結(jié)果。受試者操作特征(receiver operating characteristic,ROC)曲線分析結(jié)果顯示,術(shù)后2周時外周靜脈血中l(wèi)ncRNA PAX8-AS1、miRNA-1252-5p表達(dá)水平預(yù)測脛骨骨折內(nèi)固定術(shù)后骨折延遲愈合的ROC曲線下面積分別為0.728(P=0.000)和0.679(P=0.000),靈敏度分別為74.76%和53.33%,特異度分別為62.50%和75.00%。結(jié)論:脛骨骨折內(nèi)固定術(shù)后2周時,外周靜脈血中l(wèi)ncRNA PAX8-AS1表達(dá)水平升高和miRNANA-1252-5p表達(dá)水平降低,對預(yù)測脛骨骨折內(nèi)固定術(shù)后骨折延遲愈合具有一定的價值。
Abstract:
Objective:To explore the effects of expression levels of long non-coding RNA(lncRNA)PAX8-AS1 and microRNA(miRNA)-1252-5p in perioperative peripheral venous blood(PVB)on fracture healing after internal fixation for treatment of tibial fractures.Methods:The patients who underwent internal fixation with steel plate or intramedullary nail for treatment of tibial fractures by the same surgeons in Yixing People's Hospital from January 2018 to December 2020 were selected as the subjects,and their information including gender,age,fracture type,injury type,surgery method,smoking history,drinking history,hypertension history and diabetes mellitus history was collected.The patients were followed up and the fracture healing was observed.The expression levels of lncRNA PAX8-AS1 and miRNA-1252-5p in PVB were detected by using fluorescent quantitative PCR method before the surgery and at postsurgical week 1,2,4 and 8.According to the fracture healing at postsurgical month 4,the included patients were divided into normal healing group and delayed healing group.The general data of patients and the presurgical and postsurgical expression levels of lncRNA PAX8-AS1 and miRNA-1252-5p in PVB were compared between the 2 groups.Moreover,the factors influencing fracture healing were analyzed,and the value of the expression levels of lncRNA PAX8-AS1 and miRNA-1252-5p in PVB in predicting delayed fracture healing was evaluated.Results:①Two hundred and fifty patients were enrolled in the study,210 cases in normal healing group and 40 cases in delayed healing group.The proportions of combined diabetes mellitus,open fracture and high-energy injury were higher in delayed healing group compared to normal healing group(χ2=28.245,P=0.000; χ2=24.881,P=0.000; χ2=34.469,P=0.000).②The expression level of lncRNA PAX8-AS1 in PVB was higher and the expression level of miRNA-1252-5p in PVB was lower in delayed healing group compared to normal healing group at postsurgical week 2 and 4(t=4.742,P=0.000; t=2.753,P=0.006; t=4.079,P=0.000; t=2.538,P=0.012).③The factors influencing fracture healing after internal fixation for treatment of tibial fractures were analyzed by using generalized estimating equation(GEE),and the results showed that open fracture,high-energy injury as well as increased expression level of lncRNA PAX8-AS1 and decreased expression level of miRNA-1252-5p in PVB at postsurgical week 2 were the factors influencing fracture healing after internal fixation for tibial fractures(β=29.382,Wald χ2=8.589,P=0.000; β=22.771,Wald χ2=9.766,P=0.000; β=57.418,Wald χ2=13.735,P=0.000; β=-81.524,Wald χ2=2.392,P=0.000).④The applied value of expression levels of lncRNA PAX8-AS1 and miRNA-1252-5p in PVB in predicting delayed fracture healing after internal fixation for treatment of tibial fractures was evaluated and analyzed by using receiver operating characteristic(ROC)curve,and the results showed that,at postsurgical week 2,the areas under the ROC curves were 0.728(P=0.000)and 0.679(P=0.000),the sensitivities were 74.76% and 53.33%,and the specificities were 62.50% and 75.00% respectively.Conclusion:At postsurgical week 2,the increased expression level of lncRNA PAX8-AS1 and the decreased expression level of miRNA-1252-5p in PVB have a certain application value in prediction of delayed fracture healing after internal fixation for treatment of tibial fractures.
