摘要:
目的:評(píng)價(jià)前交叉韌帶(anterior cruciate ligament,ACL)重建術(shù)聯(lián)合富血小板血漿(platelet rich plasma,PRP)關(guān)節(jié)腔注射治療ACL損傷的臨床療效和安全性。方法:應(yīng)用計(jì)算機(jī)檢索PubMed、Cochrane Library、Embase、Web of Science、中國(guó)知網(wǎng)、維普網(wǎng)和萬(wàn)方數(shù)據(jù)庫(kù)中關(guān)于ACL重建術(shù)聯(lián)合PRP關(guān)節(jié)腔注射治療ACL損傷的隨機(jī)對(duì)照試驗(yàn)文獻(xiàn),檢索時(shí)限均為建庫(kù)至2020年12月。試驗(yàn)組采用ACL重建術(shù)聯(lián)合PRP關(guān)節(jié)腔注射治療,對(duì)照組采用ACL重建術(shù)治療。由2名研究者根據(jù)納入和排除標(biāo)準(zhǔn)獨(dú)立進(jìn)行文獻(xiàn)篩選和數(shù)據(jù)提取,意見(jiàn)不一致時(shí)討論解決。采用Cochrane手冊(cè)中的偏倚風(fēng)險(xiǎn)評(píng)估工具對(duì)納入文獻(xiàn)的質(zhì)量進(jìn)行評(píng)估,采用RevMan5.3軟件進(jìn)行數(shù)據(jù)統(tǒng)計(jì)分析。結(jié)果:共檢索到585篇文獻(xiàn),通過(guò)逐層篩選,最終納入12篇文獻(xiàn),其中中文文獻(xiàn)8篇,英文文獻(xiàn)4篇,共涉及681例患者。Meta分析結(jié)果顯示,試驗(yàn)組和對(duì)照組患者膝關(guān)節(jié)疼痛視覺(jué)模擬量表評(píng)分、Lysholm膝關(guān)節(jié)評(píng)分、國(guó)際膝關(guān)節(jié)文獻(xiàn)委員會(huì)膝關(guān)節(jié)評(píng)分、脛骨相對(duì)前移距離KT1000/2000測(cè)量值、韌帶重建MRI評(píng)分、股骨隧道直徑、脛骨隧道直徑、骨隧道直徑(將股骨隧道直徑和脛骨隧道直徑合并)的組間差異均無(wú)統(tǒng)計(jì)學(xué)意義[MD=0.22,95%CI(-0.14,0.57),P=0.230; MD=1.61,95%CI(-0.73,3.95),P=0.180; MD=2.32,95%CI(-0.92,5.55),P=0.160; MD=0.08,95%CI(-0.98,1.15),P=0.880; MD=0.24,95%CI(-0.04,0.52),P=0.090; MD=-0.25,95%CI(-1.10,0.61),P=0.57; MD=0.17,95%CI(-0.77,1.10),P=0.730; MD=-0.08,95%CI(-0.85,0.69),P=0.840]。結(jié)論:采用ACL重建術(shù)聯(lián)合PRP關(guān)節(jié)腔注射與采用ACL重建術(shù)治療ACL損傷的臨床療效和安全性相當(dāng)。
Abstract:
Objective:To evaluate the clinical efficacy and safety of anterior cruciate ligament(ACL)reconstruction(ACLR)combined with intra-articular injection of platelet-rich plasma(PRP)for treatment of ACL injury.Methods:All the randomized controlled trial(RCT)articles about ACLR combined with intra-articular injection of PRP for treatment of ACL injury included from database establishing to December 2020 were retrieved from the PubMed,Cochrane Library,Embase,Web of Science,China National Knowledge Internet,Vip Database and Wanfang Database through computer.The patients in experimental group were treated with ACLR combined with intra-articular injection of PRP,while the ones in control group with ACLR alone.The articles were screened and the information was extracted independently by two researchers according to the inclusion and exclusion criteria,and if any disagreement was found between them,discussion was conducted for making a final decision.The methodological quality of research in the articles was evaluated by using bias risk assessment tools recommended by Cochrane handbook,and then a Meta-analysis was conducted by using RevMan5.3 software.Results:Five hundred and eighty-five articles were searched out.After screening,12 articles(681 patients)were included in the final analysis,including 8 Chinese articles and 4 English articles.The results of Meta-analysis revealed that there was no statistical difference in knee pain visual analogue scale(VAS)score,Lysholm knee score,International Knee Documentation Committee(IKDC)knee score,value of relative forward distance of tibia measured by KT1000/2000,MRI scores of ligament reconstruction,femoral tunnel diameter,tibial tunnel diameter and merged bone tunnel diameter between experimental group and control group(MD=0.22,95%CI(-0.14,0.57),P=0.230; MD=1.61,95%CI(-0.73,3.95),P=0.180; MD=2.32,95%CI(-0.92,5.55),P=0.160; MD=0.08,95%CI(-0.98,1.15),P=0.880; MD=.24,95%CI(-0.04,0.52),P=0.090; MD=-0.25,95%CI(-1.10,0.61),P=0.57; MD=0.17,95%CI(-0.77,1.10),P=0.730; MD=-0.08,95%CI(-0.85,0.69),P=0.840).Conclusion:ACLR combined with intra-articular injection of PRP is similar to ACLR in clinical efficacy and safety in treatment of ACL injury.
