摘要:
目的:觀察微創(chuàng)經(jīng)皮鋼板內(nèi)固定術(shù)(minimally invasive percutaneous plate osteosynthesis,MIPPO)治療老年肱骨干長斜形骨折的臨床療效及安全性。方法:2016年1月至2019年6月,采用MIPPO治療肱骨干骨折患者42例。男26例,女16例。年齡 65~85歲,中位數(shù)72歲。均為閉合性長斜形骨折; 左側(cè)28例,右側(cè)14例; 中上段骨折30例,中下段骨折12例。根據(jù)AO骨折分型標準,A型9例、B型26例、C型7例。受傷至手術(shù)時間1~7 d,中位數(shù)3.5 d。隨訪觀察骨折愈合及并發(fā)癥發(fā)生情況,測定肩、肘關(guān)節(jié)活動度及握力。末次隨訪時,分別采用美國加州大學(xué)洛杉磯分校(the University of California at Los Angeles,UCLA)肩關(guān)節(jié)評分標準和Mayo肘關(guān)節(jié)評分標準評定肩關(guān)節(jié)和肘關(guān)節(jié)功能。結(jié)果:所有患者均獲隨訪,隨訪時間12~24個月,中位數(shù)17.5個月。骨折均骨性愈合,愈合時間3~5個月,中位數(shù)3.5個月。術(shù)后1個月,肩關(guān)節(jié)前屈活動度150.0°±7.5°、外展活動度135.0°±5.3°、外展外旋活動度50.5°±2.2°、外展內(nèi)旋活動度59.5°±4.2°,肘關(guān)節(jié)屈曲活動度105.0°±3.5°; 末次隨訪時,肩關(guān)節(jié)前屈活動度172.5°±5.5°、外展活動度169.0°±4.2°、外展外旋活動度62.5°±3.7°、外展內(nèi)旋活動度70.5°±2.1°,肘關(guān)節(jié)屈曲活動度136.5°±7.2°。末次隨訪時,患側(cè)握力(223±15)N、健側(cè)握力(231±11)N,肘關(guān)節(jié)內(nèi)外翻畸形3.2°±0.7°、前后成角3.4°±0.5°; UCLA肩關(guān)節(jié)評分(32.1±1.5)分,優(yōu)35例、良7例; Mayo肘關(guān)節(jié)評分(93.5±1.7)分,均為優(yōu)。2例術(shù)中復(fù)位失敗,行骨折端有限切開輔助復(fù)位。均未發(fā)生感染、內(nèi)固定失效、骨折移位、骨不連、橈神經(jīng)損傷等并發(fā)癥。結(jié)論:采用MIPPO治療老年肱骨干長斜形骨折,骨折愈合好,有利于肩、肘關(guān)節(jié)功能恢復(fù),且并發(fā)癥少。
