摘要:
目的:評價干細(xì)胞移植聯(lián)合髓芯減壓治療早中期股骨頭壞死(osteonecrosis of the femoral head,ONFH)的臨床療效。方法:應(yīng)用計算機檢索萬方、中國知網(wǎng)、維普、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫及PubMed、Springer、Embase數(shù)據(jù)庫中關(guān)于干細(xì)胞移植聯(lián)合髓芯減壓治療早中期ONFH的隨機對照臨床研究文獻(xiàn),檢索時限為建庫至2020年6月。試驗組采用干細(xì)胞移植聯(lián)合髓芯減壓治療ONFH,對照組采用單純髓芯減壓治療。由2名研究者根據(jù)納入和排除標(biāo)準(zhǔn)獨立檢索、篩選文獻(xiàn)并提取資料,采用Cochrane偏倚風(fēng)險評估工具對納入文獻(xiàn)的質(zhì)量進行評估,采用RevMan5.3軟件進行Meta分析。結(jié)果:共檢索出587篇文獻(xiàn),最終納入13篇文獻(xiàn)。Meta分析結(jié)果表明,試驗組術(shù)后Harris髖關(guān)節(jié)評分高于對照組[I2=45%,P=0.080; MD=11.80,95%CI(10.66,12.94)],西安大略和麥克馬斯特大學(xué)骨關(guān)節(jié)炎指數(shù)低于對照組[I2=0%,P=0.650; MD=-17.87,95%CI(-19.34,-16.40)],術(shù)后髖關(guān)節(jié)疼痛視覺模擬量表評分低于對照組[I2=86%,P=0.000; MD=-1.32,95%CI(-1.60,-1.05)],術(shù)后股骨頭嚴(yán)重塌陷或關(guān)節(jié)退行性變的發(fā)生率低于對照組[I2=0%,P=0.960; OR=0.18,95%CI(0.08,0.39)],術(shù)后股骨頭壞死區(qū)面積占股骨頭面積的百分比低于對照組[I2=0%,P=0.790; MD=-5.44,95%CI(-6.44,-4.45)]。結(jié)論:現(xiàn)有的證據(jù)表明,干細(xì)胞移植聯(lián)合髓芯減壓治療早中期ONFH的臨床療效優(yōu)于單純髓芯減壓。
Abstract:
Objective:To evaluate the clinical curative effects of stem cell transplantation(SCT)combined with core decompression(CD)for treatment of early-middle osteonecrosis of the femoral head(ONFH).Methods:All the randomized controlled trial(RCT)articles about SCT combined with CD for treatment of early-middle ONFH included from database establishing to June 2020 were retrieved from Wanfang Database,China National Knowledge Internet,VIP Database,Chinese Biomedical Literature Database,PubMed,Springer and Embase through computer.The patients in experimental group were treated with SCT and CD,while the ones in control group were merely treated with CD.The articles were retrieved and screened and the information was extracted independently by two searchers according to the inclusion and exclusion criteria.The methodological quality of research in the articles was evaluated by using Cochrane risk bias assessment tools and a Meta-analysis was conducted by using RevMan5.3 software.Results:Five hundred and eighty-seven articles were searched out.After screening,13 articles were included in the final analysis.The results of Meta-analysis revealed that the postoperative Harris hip scores were higher,while the Western Ontario and McMaster Universities(WOMAC)osteoarthritis indexes,the postoperative hip pain visual analogue scale(VAS)scores,incidence of severe collapse of femoral head or degeneration of joint and proportion of femoral head necrotic area to whole femoral head area were lower in experimental group compared to control group(I2=45%,P=0.080; MD=11.80,95%CI(10.66,12.94); I2=0%,P=0.650; MD=-17.87,95%CI(-19.34,-16.40); I2=86%,P=0.000; MD=-1.32,95%CI(-1.60,-1.05); I2=0%,P=0.960; OR=0.18,95%CI(0.08,0.39); I2=0%,P=0.790; MD=-5.44,95%CI(-6.44,-4.45)).Conclusion:Available evidences suggest that the clinical curative effect of combination therapy of SCT and CD is better than that of monotherapy of CD in treatment of early-middle ONFH.
