摘要:
目的:探討氟脫氧葡萄糖F18(fluorodeoxyglucose F18,18F-FDG)正電子發(fā)射斷層掃描及電腦斷層掃描(positron emission tomography and computed tomography,PET/CT)在未知原發(fā)灶腫瘤骨轉移(bone metastasis of unknown primary origin,BMUO)診斷中的價值。方法:選擇行18F-FDG PET/CT檢查的223例未確定原發(fā)灶的轉移性骨腫瘤患者的病例資料進行分析。男122例,女101例。年齡40~89歲,中位數(shù)62歲。所有納入研究的患者均在常規(guī)影像檢查發(fā)現(xiàn)腫瘤骨轉移病灶后3 d內行18F-FDG PET/CT檢查,查找原發(fā)腫瘤,并通過穿刺、內鏡檢查、手術等方式取病變組織標本進行組織病理檢查。18F-FDG PET/CT檢查均采用Siemens Biograph Truepoint 64(52環(huán))PET/CT。結果:在223例患者中,203例經(jīng)18F-FDG PET/CT檢查診斷為陽性,其中真陽性199例、假陽性4例; 20例診斷為陰性,其中真陰性14例、假陰性6例。18F-FDG PET/CT確定BMUO原發(fā)腫瘤的靈敏度為97.1%、特異度為77.8%、準確度為91.9%、陽性預測值為98.0%、陰性預測值為70.0%。假陽性共4例,其中結核2例、結節(jié)病1例、POEMS綜合征 1例; 假陰性6例,其中胃印戒細胞癌1例、腎透明細胞癌1例、甲狀腺癌1例、前列腺癌1例、膀胱癌2例。 18F-FDG PET/CT檢查檢測到了所有先前已知的轉移灶,并在173例患者身上發(fā)現(xiàn)了新的轉移灶。最終確診的205例患者中,肺癌109例(53.2%)、結腸癌16例(7.8%)、食管癌13例(6.3%)、前列腺癌9例(4.4%)、乳腺癌9例(4.4%)、胃癌8例(3.9%)、卵巢癌7例(3.4%)、直腸癌7例(3.4%)、鼻咽癌6例(2.9%)、喉癌5例(2.4%)、肝癌4例(2.0%)、胰腺癌4例(2.0%)、腎癌3例(1.5%)、膀胱癌2例(1.0%)、多發(fā)骨髓瘤2例(1.0%)、甲狀腺癌1例(0.5%)。按照腫瘤的組織病理學類型,腺癌共135例,約占65.8%(135/205)。結論:18F-FDG PET/CT是診斷BMUO原發(fā)腫瘤的一種可靠方法,可作為查找BMUO原發(fā)病灶的首選檢查方法。
