摘要:
目的:評價(jià)推拿治療急性腰扭傷的臨床療效和安全性。方法:計(jì)算機(jī)檢索中國知網(wǎng)、萬方數(shù)據(jù)庫、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫、維普網(wǎng)、PubMed數(shù)據(jù)庫、Cochrane Library從建庫至2018年10月收錄的推拿治療急性腰扭傷的中英文隨機(jī)對照臨床試驗(yàn)文獻(xiàn)。由2名研究者獨(dú)立進(jìn)行文獻(xiàn)篩選與數(shù)據(jù)提取。對納入文獻(xiàn)進(jìn)行質(zhì)量評價(jià)后,采用RevMan5.3軟件進(jìn)行Meta分析。結(jié)果:初檢共檢出文獻(xiàn)788篇,篩選后最終納入文獻(xiàn)11篇,均為中文文獻(xiàn)。納入的11篇文獻(xiàn)中8篇提及隨機(jī),均未報(bào)告分配隱藏、盲法、隨訪和脫落的相關(guān)信息,均報(bào)告了預(yù)設(shè)結(jié)局指標(biāo)。Meta分析結(jié)果顯示,推拿組治療總有效率、治愈率均高于對照組[OR=5.47,95%CI(3.27,9.16); OR=3.78,95%CI(2.34,6.11)],治療后腰部疼痛視覺模擬量表評分低于對照組[WMD=-2.24,95%CI(-2.36,-2.21)]。僅1篇文獻(xiàn)提及了不良反應(yīng),且2組患者均未發(fā)生不良反應(yīng),故未進(jìn)行合并分析。依次剔除文獻(xiàn)行敏感性分析,結(jié)果無明顯改變。基于治療總有效率繪制漏斗圖,圖形左右不對稱,提示可能存在發(fā)表偏倚。結(jié)論:推拿治療急性腰扭傷可緩解患者腰部疼痛,療效好,并發(fā)癥少。
Abstract:
Objective:To evaluate the clinical curative effects and safety of TUINA for treatment of acute lumbar sprain.Methods:All Chinese and English randomized controlled trial articles about TUINA for treatment of acute lumbar sprain included from database establishing to October 2018 were retrieved from China national knowledge internet,WanFang Database,Chinese biomedical literature database,VIP Database,PubMed Database and Cochrane Library through computer.The articles were screened and the data were extracted independently by two searchers.The methodological quality of research in the articles was evaluated and a Meta-analysis was conducted by using RevMan5.3 software.Results:Seven hundred and eighty-eight articles were searched out in the initial stage.After screening,11 Chinese articles were included in the final analysis.Randomization was mentioned in 8 articles out of the 11 included articles.Presupposed outcome indicators were described and allocation concealment,blind method and information about follow-up and drop-out were undescribed in all of the included articles.The results of Meta-analysis demonstrated that total effective rate and cure rate were higher and the posttreatment low back pain visual analogue scale(VAS)scores were lower in TUINA group compared to control group(OR=5.47,95%CI(3.27,9.16); OR=3.78,95%CI(2.34,6.11); WMD=-2.24,95%CI(-2.36,-2.21)).The adverse reactions were mentioned in 1 article and no adverse reactions were found in the 2 groups,so merged analysis could not be conducted.The sensitivity analysis was conducted after excluding the articles in turn,and no obvious change of meta-analysis results were found.Funnel plot was drawn according to total effective rate,and the asymmetrical funnel plot demonstrated that there might be publication bias.Conclusion:TUINA can relieve the low back pain in treatment of acute lumbar sprain,and it has the advantages of better clinical curative effects and less complications.
