摘要:
目的:分析中藥薰蒸治療腰椎間盤突出癥(lumbar disc herniation,LDH)的研究現(xiàn)狀。方法:應(yīng)用計(jì)算機(jī)檢索中國知網(wǎng)、萬方數(shù)據(jù)庫2000年1月1日至2018年12月31日收錄的所有中藥薰蒸治療LDH的文獻(xiàn)。由2位研究者分別采用Note Express3.2.0和Excel2010軟件對納入研究文獻(xiàn)的發(fā)表年份、文獻(xiàn)類型、作者和所在單位、被引頻次、基金資助情況、研究類型及主要研究內(nèi)容等數(shù)據(jù)進(jìn)行提取和統(tǒng)計(jì)。結(jié)果:共納入704篇文獻(xiàn),2000—2018年每年發(fā)表的相關(guān)文獻(xiàn)量總體呈上升趨勢,其中2007—2018年發(fā)表文獻(xiàn)數(shù)量增長較快。納入的文獻(xiàn)中學(xué)位論文14篇,期刊論文690篇。704篇文獻(xiàn)的第一作者分布于30個(gè)省、自治區(qū)、直轄市的541個(gè)單位,單位以醫(yī)療機(jī)構(gòu)為主(509個(gè)); 發(fā)表文獻(xiàn)數(shù)量排名前5的省份為河南省(76篇)、廣東省(65篇)、山東省(50篇)、湖北省(48篇)、江蘇省(41篇)、浙江省(41篇),共計(jì)321篇(45.6%)。合著文獻(xiàn)(作者人數(shù)≥2人)351篇,基金資助文獻(xiàn)34篇。被引頻次≥6次的文獻(xiàn)127篇,被引頻次≤1次的文獻(xiàn)355篇。納入研究的文獻(xiàn)以試驗(yàn)研究為主,共644篇。對文獻(xiàn)主要研究內(nèi)容的分析顯示,研究對象均缺乏嚴(yán)格限定,干預(yù)方法以綜合療法為主,具體方案差異大,觀察指標(biāo)以中醫(yī)療效、腰椎功能及疼痛評分等主觀指標(biāo)為主。結(jié)論:中藥薰蒸治療LDH的相關(guān)研究,數(shù)量多但質(zhì)量不高。今后應(yīng)廣泛開展多中心、大樣本、高質(zhì)量的RCT,基于可靠的循證醫(yī)學(xué)證據(jù)進(jìn)一步規(guī)范和優(yōu)化中藥薰蒸治療LDH的方案。
Abstract:
Objective:To analyze the current research status of Chinese herbal steaming therapy for treatment of lumbar disc herniation(LDH).Methods:All articles about Chinese herbal steaming therapy for treatment of LDH included from January 1,2000 to December 31,2018 were retrieved from China national knowledge internet and WanFang Data through computer.The information of the articles such as publication year,article type,author and author’s affiliations,citation frequency,funding,research type and main research content was extracted and statistically analyzed by two searchers by using Note Express 3.2.0 software and Excel 2010 software respectively.Results:Seven hundred and four articles were included in the final analysis,including 14 academic dissertations and 690 journal articles,351 co-authored articles(number of author≥2)and 34 funded articles,127 articles with citation frequency≥6 times and 355 articles with citation frequency≤1 time.The annual number of relevant articles presented a rising trend in general in 2000-2018 and increased rapidly in 2007-2018.The first authors of 704 articles came from 541 units in 30 provinces,autonomous regions and direct-controlled municipality,most of which were mainly medical institutions(509).The top 5 provinces with large number of published articles included Henan(76),Guangdong(65),Shandong(50),Hubei(48),Jiangsu(41)and Zhejiang(41)in turn,and they provided 321(45.6%)articles in total.Six hundred and forty-four articles,which were mainly experimental studies,were included in the final analysis.The results of analysis on main research contents in articles demonstrated that(1)the research objects were selected without strict restrictions;(2)the intervention methods were mainly comprehensive therapies;(3)there were large differences between specific schemes and(4)the observation indexes were mainly subjective indexes such as TCM curative effect,lumbar function and pain scores.Conclusion:There was a large number of articles with low quality about Chinese herbal steaming therapy for treatment of LDH.The multi-center,large-sample and high-quality RCTs need to be widely developed in the future,and the scheme about Chinese herbal steaming therapy for treatment of LDH need to be further standardized and optimized based on reliable evidence of evidence-based medicine.
