摘要:
目的:對(duì)氨基葡萄糖聯(lián)合塞來(lái)昔布口服治療膝骨關(guān)節(jié)炎(knee osteoarthritis,KOA)的臨床療效進(jìn)行系統(tǒng)評(píng)價(jià)。方法:通過(guò)計(jì)算機(jī)檢索中國(guó)知網(wǎng)、中國(guó)生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(kù)、維普醫(yī)藥信息資源系統(tǒng)、萬(wàn)方數(shù)據(jù)庫(kù)、PubMed及EMBase數(shù)據(jù)庫(kù)從1995年1月至2018年12月收錄的有關(guān)氨基葡萄糖聯(lián)合塞來(lái)昔布口服(治療組)和單純?nèi)麃?lái)昔布口服(對(duì)照組)治療KOA的隨機(jī)對(duì)照臨床研究文獻(xiàn),篩選文獻(xiàn)、提取資料,并采用Jadad量表對(duì)文獻(xiàn)質(zhì)量進(jìn)行評(píng)價(jià)后,進(jìn)行Meta分析。結(jié)果:初檢共檢索到相關(guān)文獻(xiàn)94篇,篩選后最終納入文獻(xiàn)9篇,均為中文文獻(xiàn)。納入文獻(xiàn)均為單中心隨機(jī)對(duì)照臨床試驗(yàn),共納入KOA患者882例。9篇文獻(xiàn)Jadad量表評(píng)分均≤2分,文獻(xiàn)質(zhì)量偏低。Meta分析結(jié)果顯示,治療組治療有效率高于對(duì)照組[OR=8.51,95%CI(4.10,17.64)],治療后患膝Lequesne骨關(guān)節(jié)炎嚴(yán)重度指數(shù)和疼痛視覺模擬量表評(píng)分均低于對(duì)照組[WMD=-1.59,95%CI(-2.16,-1.03); WMD=-1.55,95%CI(-2.27,-0.83)]。結(jié)論:氨基葡萄糖聯(lián)合塞來(lái)昔布口服是治療KOA的有效方法,其療效優(yōu)于單純?nèi)麃?lái)昔布口服。但由于研究所納入的文獻(xiàn)質(zhì)量不高,該療法的療效還需多中心、大樣本的臨床隨機(jī)對(duì)照試驗(yàn)進(jìn)一步證實(shí)。
Abstract:
Objective:To systematically review the clinical curative effects of oral applications of glucosamine and celecoxib for treatment of knee osteoarthritis(KOA).Methods:All the randomized controlled trial(RCT)articles about combination therapy of oral applications of glucosamine and celecoxib(treatment group)versus monotherapy of oral application of celecoxib(control group)for treatment of KOA included from January 1995 to December 2018 were retrieved from China national knowledge internet,Chinese biomedical literature database,VMIS,WanFang Database,PubMed and EMBase Database through computer.The RCT articles were screened and the information was extracted.The methodological quality of research in the articles was evaluated by using Jadad scale and a Meta-analysis was conducted.Results:Ninety-four articles were searched out.After screening,9 Chinese single-center RCT articles(882 patients with KOA)were included in the final analysis.The Jadad scores of the 9 articles were≤2 points,and it suggested that the quality of these articles was poor.The results of Meta-analysis demonstrated that the effective rates were higher and the posttreatment Lequesne knee osteoarthritis severity index and pain visual analogue scale scores were lower in treatment group compared to control group(OR=8.51,95%CI(4.10,17.64); WMD=-1.59,95%CI(-2.16,-1.03); WMD=-1.55,95%CI(-2.27,-0.83)).Conclusion:The combination therapy of oral applications of glucosamine and celecoxib is an effective method for treatment of KOA,and it surpasses the monotherapy of oral application of celecoxib in the clinical curative effect.However,its clinical curative effect need to be further verified by multi-center,large sample,high-quality RCTs.
