摘要:
目的:觀察韋氏脊柱整治手法結(jié)合核心肌群訓(xùn)練治療非職業(yè)高爾夫運(yùn)動者非特異性腰痛的臨床療效。方法:將66例患有非特異性腰痛的非職業(yè)高爾夫運(yùn)動者隨機(jī)分為推拿綜合組、手法綜合組和強(qiáng)化訓(xùn)練組,每組22例。推拿綜合組采用推拿手法聯(lián)合場效應(yīng)治療,手法綜合組采用韋氏脊柱整治手法聯(lián)合場效應(yīng)治療,強(qiáng)化訓(xùn)練組采用韋氏脊柱整治手法聯(lián)合場效應(yīng)治療和核心肌群訓(xùn)練; 每周治療3次,10次為1個療程,共3個療程。分別于治療前、治療結(jié)束后1個月和3個月記錄并比較3組患者改良Oswestry功能障礙指數(shù)(Oswestry disability index,ODI),并于治療結(jié)束后6個月采用自擬標(biāo)準(zhǔn)評價3組患者運(yùn)動能力恢復(fù)情況。結(jié)果:①改良ODI。時間因素和分組因素存在交互效應(yīng)(F=7.803,P=0.000); 3組患者改良ODI比較,組間差異有統(tǒng)計學(xué)意義,即存在分組效應(yīng)(F=22.786,P=0.000); 治療前后不同時間點之間改良ODI的差異有統(tǒng)計學(xué)意義,即存在時間效應(yīng)(F=399.125,P=0.000); 3組患者改良ODI隨時間均呈降低趨勢,但3組的降低趨勢不完全一致[(69.39±7.35)%,(32.72±14.58)%,(19.80±11.43)%,F=191.759,P=0.000;(69.09±10.87)%,(44.34±11.93)%,(29.80±7.87)%,F=84.015,P=0.000;(73.13±10.73)%,(55.96±6.91)%,(37.58±6.36)%,F=187.706,P=0.000]; 治療前3組患者改良ODI比較,差異無統(tǒng)計學(xué)意義(F=1.041,P=0.359); 治療結(jié)束后1個月,3組患者改良ODI比較,差異有統(tǒng)計學(xué)意義(F=22.114,P=0.000); 強(qiáng)化訓(xùn)練組改良ODI低于手法綜合組和推拿綜合組(P=0.001,P=0.000),手法綜合組改良ODI低于推拿綜合組(P=0.001); 治療結(jié)束后3個月,3組患者改良ODI比較,差異有統(tǒng)計學(xué)意義(F=22.495,P=0.000); 強(qiáng)化訓(xùn)練組改良ODI低于手法綜合組和推拿綜合組(P=0.000,P=0.000),手法綜合組改良ODI低于推拿綜合組(P=0.005)。②運(yùn)動能力恢復(fù)效果。治療結(jié)束后6個月,強(qiáng)化訓(xùn)練組優(yōu)4例、良8例、可8例、差2例,手法綜合組優(yōu)1例、良4例、可6例、差11例,推拿綜合組良2例、可2例、差18例; 強(qiáng)化訓(xùn)練組運(yùn)動恢復(fù)效果優(yōu)于手法綜合組和推拿綜合組(R^-強(qiáng)化訓(xùn)練組=19.73,R^-手法綜合組=35.41,R^-推拿綜合組=45.36,χ2=20.497,P=0.000)。結(jié)論:采用韋氏脊柱整治手法聯(lián)合場效應(yīng)治療和核心肌群訓(xùn)練、韋氏脊柱整治手法聯(lián)合場效應(yīng)治療、推拿手法聯(lián)合場效應(yīng)治療,均能緩解非職業(yè)高爾夫運(yùn)動者的非特異性腰痛,改善腰椎功能,提高運(yùn)動能力,但前者的療效優(yōu)于后兩者。
Abstract:
ABSTRACT Objective:To observe the clinical curative effects of Wei's spinal remediation manipulation combined with core muscle training for treatment of nonspecific low back pain(NLBP)in non-professional golfers.Methods:Sixty-six non-professional golfers with NLBP were randomly divided into TUINA comprehensive group,manipulation comprehensive group and intensive training group,22 cases in each group.The patients in TUINA comprehensive group and manipulation comprehensive group were treated with TUINA manipulation combined with field effect therapy and Wei's spinal remediation manipulation combined with field effect therapy respectively,and the patients in intensive training group were treated with Wei's spinal remediation manipulation combined with field effect therapy and core muscle training,three times a week for consecutive 3 courses of treatment,ten times for each course.The improved Oswestry disability index(ODI)were recorded and compared between the 3 groups before treatment and at 1 and 3 months after the end of the treatment respectively,and the athletic ability recovery was also evaluated by using self-made evaluation standard at 6 months after the end of the treatment.Results:There was interaction between time factor and group factor in improved ODI(F=7.803,P=0.000).There was statistical difference in improved ODI between the 3 groups,in other words,there was group effect(F=22.786,P=0.000).There was statistical difference in improved ODI between different timepoints before and after the treatment,in other words,there was time effect(F=399.125,P=0.000).The improved ODI presented a time-dependent decreasing trend in the 3 groups,while the 3 groups were inconsistent with each other in the decreasing trend of improved ODI(69.39+/-7.35,32.72+/-14.58,19.80+/-11.43%,F=191.759,P=0.000; 69.09+/-10.87,44.34+/-11.93,29.80+/-7.87%,F=84.015,P=0.000; 73.13+/-10.73,55.96+/-6.91,37.58+/-6.36%,F=187.706,P=0.000).There was no statistical difference in improved ODI between the 3 groups before the treatment(F=1.041,P=0.359).There was statistical difference in improved ODI between the 3 groups at 1 month after the end of the treatment(F=22.114,P=0.000).The improved ODI was lower in intensive training group compared to manipulation comprehensive group and TUINA comprehensive group(P=0.001,P=0.000),and was lower in manipulation comprehensive group compared to TUINA comprehensive group(P=0.001).There was statistical difference in improved ODI between the 3 groups at 3 months after the end of the treatment(F=22.495,P=0.000).The improved ODI was lower in intensive training group compared to manipulation comprehensive group and TUINA comprehensive group(P=0.000,P=0.000),and was lower in manipulation comprehensive group compared to TUINA comprehensive group(P=0.005).At 6 months after the end of the treatment,4 patients obtained an excellent result of athletic ability recovery,8 good,8 fair and 2 poor in intensive training group; 1 patient obtained an excellent result of athletic ability recovery,4 good,6 fair and 11 poor in manipulation comprehensive group; and 2 patients obtained a good result of athletic ability recovery,2 fair and 18 poor in TUINA comprehensive group.The intensive training group surpassed the manipulation comprehensive group and the TUINA comprehensive group in the result of athletic ability recovery(R^-intensive training group=19.73,R^-manipulation comprehensive group=35.41,R^-TUINA comprehensive group=45.36,χ2=20.497,P=0.000).Conclusion:The combination therapy of Wei's spinal remediation manipulation and field effect therapy and core muscle training,the combination therapy of Wei's spinal remediation manipulation and field effect therapy and the combination therapy of TUINA manipulation and field effect therapy can relieve the NLBP of non-professional golfers,and can improve their lumbar function and promote the athletic ability.However,the curative effect of the former is better than that of the latter two.
