摘要:
目的:探討電針聯(lián)合玻璃酸鈉關(guān)節(jié)腔注射治療踝關(guān)節(jié)創(chuàng)傷性關(guān)節(jié)炎(traumatic arthritis,TA)的臨床療效。方法:將符合要求的96例踝關(guān)節(jié)TA患者隨機分為2組,每組48例。聯(lián)合治療組采用電針聯(lián)合玻璃酸鈉關(guān)節(jié)腔注射治療,關(guān)節(jié)注射組采用玻璃酸鈉關(guān)節(jié)腔注射治療。電針治療每天1 次,5 d為1個療程,間隔2 d后繼續(xù)下一療程,共治療4個療程; 玻璃酸鈉關(guān)節(jié)腔注射治療每周1次,共治療4次。分別于治療前后采用Maryland足部評分標準從疼痛和功能2個方面進行療效評定。結(jié)果:至治療結(jié)束時,2組均無患者脫落。治療前2組患者的Maryland評分比較,差異無統(tǒng)計學(xué)意義[(67.76±9.17)分,(65.13±8.61)分,t=1.502,P=0.140]; 治療后2組患者的評分均增加(t=2.862,P=0.016; t=14.605,P=0.653),聯(lián)合治療組的評分高于關(guān)節(jié)注射組[(85.57±5.29)分,(78.96±8.49)分,t=5.257,P=0.000]。結(jié)論:電針聯(lián)合玻璃酸鈉關(guān)節(jié)腔注射治療能有效減輕踝關(guān)節(jié)TA患者的疼痛癥狀,改善踝關(guān)節(jié)功能,療效優(yōu)于單純玻璃酸鈉關(guān)節(jié)腔注射治療。
Abstract:
ABSTRACT Objective:To explore the clinical curative effect of electroacupuncture therapy combined with intra-articular injection of sodium hyaluronate in the treatment of traumatic arthritis(TA)of ankle joint.Methods:Ninety-six patients with ankle TA were enrolled in the study and randomly divided into 2 groups,48 cases in each group.The patients were treated with combination therapy of electroacupuncture and intra-articular injection of sodium hyaluronate(group A)and monotherapy of intra-articular injection of sodium hyaluronate(group B)respectively.The electroacupuncture therapy were performed on the patients once a day for consecutive 4 course of treatment,5 days for each course with a 2-day rest-insertion between courses.The intra-articular injection of sodium hyaluronate were performed on the patients once a week for 4 times.The curative effects on ankle pain and ankle function were evaluated by using Maryland foot scores before and after treatment respectively.Results:No patients dropped out of the 2 groups at the end of the treatment.There was no statistical difference in Maryland scores between the 2 groups before the treatment(67.76+/-9.17 vs 65.13+/-8.61 points,t=1.502,P=0.140).The Maryland scores increased in both of the 2 groups after the treatment(t=2.862,P=0.016; t=14.605,P=0.653),and the Maryland scores were higher in group A compared to group B(85.57+/-5.29 vs 78.96+/-8.49 points,t=5.257,P=0.000).Conclusion:The combination therapy of electroacupuncture and intra-articular injection of sodium hyaluronate can effectively relieve the ankle pain and improve the ankle function in patients with ankle TA,and its clinical curative effects is better than that of monotherapy of intra-articular injection of sodium hyaluronate.
