摘要:
目的:探討張氏膝痛方聯(lián)合塞來(lái)昔布膠囊口服治療濕熱阻絡(luò)型膝關(guān)節(jié)滑膜炎的臨床療效和安全性。方法:2013年1月至2016年1月,選擇濕熱阻絡(luò)型膝關(guān)節(jié)滑膜炎患者100例,隨機(jī)分為2組,每組50例。分別采用張氏膝痛方聯(lián)合塞來(lái)昔布膠囊口服治療和單純?nèi)麃?lái)昔布膠囊口服治療。4周為1個(gè)療程,共服1個(gè)療程。治療前后,依據(jù)Lysholm膝關(guān)節(jié)評(píng)分標(biāo)準(zhǔn)對(duì)2組患者膝關(guān)節(jié)進(jìn)行評(píng)分,根據(jù)治療后Lysholm膝關(guān)節(jié)評(píng)分增加的程度評(píng)價(jià)療效,并觀察服藥后不良反應(yīng)發(fā)生情況。結(jié)果:治療前,2組患者Lysholm膝關(guān)節(jié)評(píng)分比較,差異無(wú)統(tǒng)計(jì)學(xué)意義[(58.14±10.73)分,(57.72±10.97)分; t=0.195,P=0.846)]; 治療后,2組患者Lysholm膝關(guān)節(jié)評(píng)分均增加(t=-14.020,P=0.000; t=-9.545,P=0.000); 且膝痛方聯(lián)合塞來(lái)昔布組Lysholm膝關(guān)節(jié)評(píng)分高于塞來(lái)昔布組[(86.14±9.17)分,(75.30±8.31)分; t=6.194,P=0.000]。根據(jù)治療后Lysholm膝關(guān)節(jié)評(píng)分增加的程度評(píng)價(jià)療效,膝痛方聯(lián)合塞來(lái)昔布組,顯效21例、有效22例、好轉(zhuǎn)5例、無(wú)效2例; 塞來(lái)昔布組,顯效9例、有效22例、好轉(zhuǎn)10例、無(wú)效9例; 膝痛方聯(lián)合塞來(lái)昔布組臨床療效優(yōu)于塞來(lái)昔布組(Z=-3.261,P=0.001)。2組各出現(xiàn)胃腸道不良反應(yīng)1例,經(jīng)口服奧美拉唑膠囊治療后癥狀消失; 均無(wú)肝腎功能障礙等嚴(yán)重不良反應(yīng)發(fā)生。2組患者不良反應(yīng)發(fā)生率比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(χ2=0.000,P=1.000)。結(jié)論:張氏膝痛方聯(lián)合塞來(lái)昔布膠囊口服治療濕熱阻絡(luò)型膝關(guān)節(jié)滑膜炎,療效優(yōu)于單純口服塞來(lái)昔布膠囊,且并未增加不良反應(yīng)發(fā)生的風(fēng)險(xiǎn)。
Abstract:
ABSTRACT Objective:To explore the clinical curative effects and safety of oral application of Zhang's Xitong Fang(張氏膝痛方,ZSXTF)and celebrex capsules for treatment of gonarthromeningitis with DAMPNESS-HEAT BLOCK COLLATERALS syndrome.Methods:From January 2013 to January 2016,one hundred patients with gonarthromeningitis with DAMPNESS-HEAT BLOCK COLLATERALS syndrome were selected and randomly divided into 2 groups,50 cases in each group.The patients were treated with combination therapy of application of ZSXTF and celebrex capsules(group A)and monotherapy of oral application of celebrex capsules(group B)respectively for consecutive 4 weeks.The knee joints were scored by using Lysholm knee joint scoring standards before and after the treatment in both of the 2 groups,and the curative effects were evaluated according to the increasing degree of posttreatment Lysholm knee scores.The incidence rate of adverse reactions was also observed.Results:There was no statistical difference in Lysholm knee joint scores between the 2 groups before the treatment(58.14+/-10.73 vs 57.72+/-10.97 points,t=0.195,P=0.846).The Lysholm knee joint scores increased in both of the 2 groups after the treatment(t=-14.020,P=0.000; t=-9.545,P=0.000).The Lysholm knee joint scores were higher in group A compared to group B(86.14+/-9.17 vs 75.30+/-8.31 points,t=6.194,P=0.000).Twenty-one patients obtained an excellent result,22 good,5 fair and 2 poor in group A; while 9 patients obtained an excellent result,22 good,10 fair and 9 poor in group B.The group----------------------------------------------- A surpassed the group B in the total curative effect(Z=-3.261,P=0.001).The gastrointestinal adverse reactions were found in the 2 groups(1 case in each group),and the symptoms disappeared after oral application of omeprazole capsules.No severe adverse reaction such as hepatorenal disfunction were found in the 2 groups.There was no statistical difference in the incidence rate of adverse reactions between the 2 groups(χ2=0.000,P=1.000).Conclusion:The curative effect of combination therapy of oral application of ZSXTF and celebrex capsules is better than that of monotherapy of oral application of celebrex capsules in the treatment of gonarthromeningitis with DAMPNESS-HEAT BLOCK COLLATERALS syndrome,and it do not increase the risk of adverse reactions.
