摘要:
目的:觀察針刀松解治療指屈肌腱狹窄性腱鞘炎的臨床療效和安全性。方法:將89例指屈肌腱狹窄性腱鞘炎患者隨機(jī)分為2組,45例采用針刀松解治療,44例采用局部封閉治療。分別于治療后3周和治療后6個(gè)月,參照《中醫(yī)病證診斷療效標(biāo)準(zhǔn)》中指屈肌腱狹窄性腱鞘炎的療效評(píng)定標(biāo)準(zhǔn)對(duì)2組患者的臨床療效進(jìn)行評(píng)定和比較,并觀察并發(fā)癥發(fā)生情況。結(jié)果:治療后3周,針刀組顯效45例,封閉組顯效25例、好轉(zhuǎn)16例、無(wú)效3例,針刀組療效優(yōu)于封閉組(Z=-4.924,P=0.000)。治療后6個(gè)月,針刀組顯效43例、好轉(zhuǎn)2例,封閉組顯效19例、好轉(zhuǎn)18例、無(wú)效7例; 針刀組療效優(yōu)于封閉組(Z=-5.348,P=0.000)。2組患者均無(wú)神經(jīng)、肌腱損傷及組織黏連等并發(fā)癥發(fā)生。結(jié)論:針刀松解治療指屈肌腱狹窄性腱鞘炎,可有效緩解疼痛、改善患指功能,安全可靠,療效優(yōu)于局部封閉治療。
Abstract:
Objective:To observe the clinical curative effects and safety of knife needle release in the treatment of stenosing tenosynovitis of finger flexor tendon.Methods:Eighty-nine patients with stenosing tenosynovitis of finger flexor tendon were randomly divided into 2 groups,45 patients(knife needle group)were treated with percutaneous release with knife needle and 44 patients(blocking group)were treated with local blocking therapy.The curative effect were evaluated and compared between the 2 groups at 3 weeks and 6 months after the treatment respectively by using the therapeutic effects evaluation standard of stenosing tenosynovitis of finger flexor tendon which was extracted from Standard for diagnosis and therapeutic effectiveness evaluation of traditional Chinese medicine syndromes,and the complications were also observed.Results:Forty-five patients got a good result in the knife needle group,while 25 patients got a good result,16 fair and 3 poor in the blocking group at 3 weeks after the treatment.The knife needle group surpassed the blocking group in the curative effect(Z=-4.924,P=0.000).Forty-three patients got a good result and 2 fair in the knife needle group,while 19 patients got a good result,18 fair and 7 poor in the blocking group at 6 months after the treatment.The knife needle group surpassed the blocking group in the curative effect(Z=-5.348,P=0.000).No complications such as nerve injuries,tendon injuries and tissue adhesions were found in the 2 groups.Conclusion:Percutaneous release with knife needle can effectively relieve the finger pain and improve the finger function.It is safe and reliable for treatment of stenosing tenosynovitis of finger flexor tendon,and its curative effect is better than that of local blocking therapy.
