摘要:
目的:探討血管內(nèi)皮生長(zhǎng)因子(vascular endothelial growth factor,VEGF)和骨形態(tài)發(fā)生蛋白2(bone morphogenetic protein 2,BMP-2)在非創(chuàng)傷性股骨頭壞死不同區(qū)域的表達(dá)情況。方法:將從單側(cè)人工髖關(guān)節(jié)置換術(shù)中取得的40個(gè)非創(chuàng)傷性壞死股骨頭沿冠狀面自正中用擺鋸剖開,分別在壞死區(qū)、交界區(qū)和正常區(qū)各鑿取1塊約1 cm×1 cm×0.4 cm的骨塊,制成石蠟切片,采用免疫組織化學(xué)法測(cè)定不同區(qū)域VEGF和BMP-2的表達(dá)情況。結(jié)果:①VEGF表達(dá)結(jié)果。3個(gè)區(qū)域VEGF陽(yáng)性面積百分比比較,差異有統(tǒng)計(jì)學(xué)意義[(6.25±2.61)%,(19.30±4.56)%,(11.63±4.12)%; χ2=81.100,P=0.000]; 壞死區(qū)陽(yáng)性面積百分比小于交界區(qū)和正常區(qū)(χ2=81.040,P=0.000; χ2=22.200,P=0.000),交界區(qū)大于正常區(qū)(χ2=18.410,P=0.000)。3個(gè)區(qū)域VEGF表達(dá)平均灰度比較,差異有統(tǒng)計(jì)學(xué)意義(86.30±6.84,31.85±4.89,73.93±5.78; F=939.984,P=0.000); 壞死區(qū)平均灰度大于交界區(qū)和正常區(qū)(P=0.000; P=0.000),交界區(qū)小于正常區(qū)(P=0.000)。②BMP-2的表達(dá)結(jié)果。3個(gè)區(qū)域的BMP-2陽(yáng)性面積百分比比較,差異有統(tǒng)計(jì)學(xué)意義[(5.20±2.46)%,(17.43±4.77)%,(10.03±4.32)%; χ2=76.450,P=0.000]; 壞死區(qū)陽(yáng)性面積百分比小于交界區(qū)和正常區(qū)(χ2=76.320,P=0.000; χ2=21.950,P=0.000),交界區(qū)大于正常區(qū)(χ2=27.400,P=0.000)。3個(gè)區(qū)域BMP-2表達(dá)平均灰度比較,差異有統(tǒng)計(jì)學(xué)意義(88.65±6.61,41.25±5.00,76.80±5.33; F=751.985,P=0.000); 壞死區(qū)BMP-2表達(dá)平均灰度大于交界區(qū)和正常區(qū)(P=0.000; P=0.000),交界區(qū)小于正常區(qū)(P=0.000)。結(jié)論:VEGF和BMP-2在非創(chuàng)傷性股骨頭壞死交界區(qū)表達(dá)強(qiáng)度高于壞死區(qū)和正常區(qū),交界區(qū)可能是影響股骨頭壞死預(yù)后的關(guān)鍵區(qū)域。
Abstract:
Objective:To explore the expressions of vascular endothelial growth factor(VEGF)and bone morphogenetic protein 2(BMP-2)in different zones of femoral head with non-traumatic osteonecrosis.Methods:Forty non-traumatic necrotic femoral head achieved via unilateral artificial hip replacement were splited along the median coronal plane by using swing saw,and a 1.0×1.0×0.4-cm specimen was fetched from the necrotic zone,junctional zone and normal zone respectively and was made into paraffin sections.Then the expressions of VEGF and BMP-2 were detected by using immunohistochemical method.Results:There was statistical difference in the positive area percentage of VEGF between the 3 zones(6.25+/-2.61,19.30+/-4.56,11.63+/-4.12%; χ2=81.100,P=0.000),and the necrotic zone was inferior to the junctional zone and normal zone(χ2=81.040,P=0.000; χ2=22.200,P=0.000)while the junctional zone surpassed the normal zone(χ2=18.410,P=0.000).There was statistical difference in the average gray value of VEGF expression between the 3 zones(86.30+/-6.84,31.85+/-4.89,73.93+/-5.78; F=939.984,P=0.000),and the necrotic zone surpassed the junctional zone and normal zone(P=0.000; P=0.000)while the junctional zone was inferior to the normal zone(P=0.000).There was statistical difference in the positive area percentage of BMP-2 between the 3 zones(5.20+/-2.46,17.43+/-4.77,10.03+/-4.32%; χ2=76.450,P=0.000),and the necrotic zone was inferior to the junctional zone and normal zone(χ2=76.320,P=0.000; χ2=21.950,P=0.000)while the junctional zone surpassed the normal zone(χ2=27.400,P=0.000).There was statistical difference in the average gray value of BMP-2 expression between the 3 zones(88.65+/-6.61,41.25+/-5.00,76.80+/-5.33; F=751.985,P=0.000),and the necrotic zone surpassed the junctional zone and normal zone(P=0.000; P=0.000)while the junctional zone was inferior to the normal zone.Conclusion:The expression intensity of VEGF and BMP-2 of the junctional zone of non-traumatic femoral head necrosis is higher than that of necrotic zone and normal zone,therefore the junctional zone may be the key zone which will influence the prognosis of femoral head necrosis.
