摘要:
目的:分析平樂(lè)郭氏正骨醫(yī)家治療股骨頭壞死(osteonecrosis of the femoral head,ONFH)血瘀證的用藥規(guī)律。方法:收集平樂(lè)郭氏正骨醫(yī)家治療的121例ONFH血瘀證患者的門診病歷(共涉及357張臨床處方),建立診療數(shù)據(jù)庫(kù)。應(yīng)用最大頻繁關(guān)聯(lián)模式挖掘技術(shù),探討針對(duì)ONFH血瘀證癥狀的平樂(lè)郭氏正骨臨床用藥內(nèi)在規(guī)律。結(jié)果:氣滯血瘀證和痰阻血瘀證是ONFH血瘀證中較為常見的子證型; 髖痛、活動(dòng)受限是ONFH血瘀證最常見的癥狀; 桃仁和紅花組成的藥對(duì)與髖刺痛、跛行癥狀組合關(guān)系最密切; 桃仁、紅花、當(dāng)歸為活血祛瘀、通經(jīng)止痛的常用藥物組合; 瓜蔞、薤白、厚樸、柴胡、川芎為行氣止痛的常用藥物組合; 附子、桂枝為溫通散瘀的常用藥物組合; 赤芍、大黃為清熱除瘀的常用藥物組合。結(jié)論:活血化瘀藥與行氣藥配伍是平樂(lè)郭氏正骨醫(yī)家治療ONFH血瘀證最常用的藥物組合。
Abstract:
Objective:To study the medication rule of Pingle Guo's orthopedic practitioners in treamtment of BLOOD STASIS syndrome in patients with osteonecrosis of the femoral head(ONFH).Methods:The outpatient medical records(357 clinical prescriptions)of 121 patients with ONFH combined with BLOOD STASIS syndrome who were treated by Pingle Guo's orthopedic practitioners were collected to establish diagnosis and treatment database.Then maximal frequent association pattern mining technology was used to explore the clinical medication rules of Pingle Guo's orthopedics practitioners in treatment of BLOOD STASIS syndrome in patients with ONFH.Results:QI STAGNATION BLOOD STASIS syndrome and PHLEGM OBSTRUCTING BLOOD STASIS syndrome were the common subtypes of BLOOD STASIS syndrome of ONFH.Hip pain and action-limited were the most common symptoms of ONFH patients with BLOOD STASIS syndrome.The couplet medicinals composed of peach seed and safflowe had close relation to the combination of hip pain and claudication.Peach seed,safflowe and angelica sinensis were the common drug combinations for BLOOD-ACTIVATING and STASIS-DISPELLING and unblock the meridian to relieve pain.Trichosanthis,allium macrostemon,magnoliae officinalis,radix bupleuri and ligusticum CHUANXIONG were the common drug combinations for move QI to relieve pain.Monkshood and cassia twig were the common drug combinations for WARMING COLLATERALS and DISSIPATING STASIS.Red peony root and rheum officinale were the common drug combinations for CLEARING HEAT and RELIEVING STAGNATION.Conclusion:BLOOD-ACTIVATING and STASIS-RESOLVING medicine compatible with QI-ACTIVATING medicine is the common drug combination for treatment of BLOOD STASIS syndrome of patients with ONFH by Pingle Guo's orthopedic practitioners.
