摘要:
目的:探討可灌注骨水泥椎弓根螺釘系統(tǒng)在老年腰椎疾患手術(shù)中的應(yīng)用價(jià)值。方法:采用可灌注骨水泥螺釘系統(tǒng)治療老年 腰椎疾患患者26例,男7例,女19例。年齡63~75歲,中位數(shù)68.5歲。腰椎間盤突出癥10例,腰椎管狹窄9例,腰椎滑脫7例。均為單 節(jié)段病變,均合并嚴(yán)重骨質(zhì)疏松,病變節(jié)段相鄰椎體骨密度為(0.425±0.093)g·cm-3。分別于術(shù)前、術(shù)后6周、 術(shù)后3個(gè)月及末次隨訪時(shí)采用視覺模擬評分法(visual analogue score,VAS)和日本骨科協(xié)會(Japanese orthopaedic association,JOA)腰痛疾患評分標(biāo)準(zhǔn)評定患者的腰部疼痛情況和腰部功能,同時(shí)觀察治療及隨訪期間并發(fā)癥的發(fā)生情況。結(jié)果: 本組26例患者均獲隨訪,隨訪時(shí)間20~38個(gè)月,中位數(shù)25.5個(gè)月。術(shù)后2例患者發(fā)生切口感染,應(yīng)用敏感抗生素后控制; 4例發(fā)生尿 路感染,1例出現(xiàn)肝功能異常,請相關(guān)科室會診治療后恢復(fù)正常; 2例患者分別在術(shù)后6個(gè)月及術(shù)后12個(gè)月出現(xiàn)腰背部疼痛,口服非 甾體消炎鎮(zhèn)痛藥后明顯緩解。未發(fā)生內(nèi)固定松動(dòng)、斷裂、脫出等并發(fā)癥。本組患者術(shù)前、術(shù)后6周、術(shù)后3個(gè)月及末次隨訪時(shí)的 VAS評分和JOA評分分別為(5.6±1.1)分、(5.3±0.9)分、(3.1±0.6)分、(1.3±0.2)分,(11.2±3.4)分 、(16.2±3.1)分、(17.1±1.2)分、(23.1±2.4)分。結(jié)論:應(yīng)用可灌注骨水泥椎弓根螺釘系統(tǒng)固定治療老年腰椎 疾患,固定牢固,可有效緩解患者的疼痛癥狀,改善腰部功能,而且具有較高的安全性。
