摘要:
目的:探討血清網(wǎng)膜素在類風濕關節(jié)炎發(fā)病中的作用。方法:按要求納入48例類風濕關節(jié)炎患者(RA組)和45例健康志愿者(對照組)。RA組患者采用自擬加味四藤湯治療,對照組不進行任何治療。RA組患者于治療前及治療后3個月分別空腹14 h后采肘靜脈血測定網(wǎng)膜素、紅細胞沉降率、腫瘤壞死因子-α、白細胞介素-6、C反應蛋白; 對照組僅進行1次采血,并測定以上指標。以上指標的數(shù)據(jù)采集完畢后,分別將治療前RA組與對照組各項指標、RA組治療前后各項指標進行比較,同時對RA組患者血清網(wǎng)膜素含量與其余各項指標的關系進行分析。結(jié)果:①治療前RA組及對照組的臨床生化特征指標。治療前RA組腫瘤壞死因子-α、白細胞介素-6、C反應蛋白、血沉和網(wǎng)膜素的水平均高于對照組[(2.901±0.720)μg·L-1,(0.815±0.411)μg·L-1;(8.043±3.706)ng·L-1,( 2.727±1.852)ng·L-1;(19.384±7.450)mg·L-1,(8.022±2.615)mg·L-1;(23.830±8.601)mm·h-1,(12.734±5.910)mm·h-1;(30.525±3.970)μg·L-1,(25.302±4.317)μg·L-1],差異有統(tǒng)計學意義(t=-2.712,P=0.000; t=3.971,P=0.000; t=-10.562,P=0.000; t=-8.477,P=0.000; t=5.175,P=0.000)。②RA組治療前后的臨床生化特征指標。治療后RA組血清腫瘤壞死因子-α、白細胞介素-6和網(wǎng)膜素的含量降低[(1.305±0.226)μg·L-1,(6.290±2.651)ng·L-1,(18.952±3.308)mg·L-1,(22.985±3.250)mm·h-1,(26.271±3.683)μg·L-1],與治療前比較差異有統(tǒng)計學意義(t=1.905,P=0.000; t=4.991,P=0.000; t=3.226,P=0.005); 治療后C反應蛋白、血沉的水平[(18.952±3.308)mg·L-1,(22.985±3.250)mm·h-1]與治療前比較,差異無統(tǒng)計學意義(t=2.157,P=0.113; t=3.401,P=0.098)。③血清網(wǎng)膜素含量與其他臨床生化特征指標的關系。RA組網(wǎng)膜素與腫瘤壞死因子-α及白細胞介素-6呈正相關(r=0.723,P=0.011; r=0.480,P=0.027)。以血清網(wǎng)膜素為因變量,以血沉、C反應蛋白、白細胞介素-6、腫瘤壞死因子-α為自變量進行多元回歸分析,結(jié)果表明腫瘤壞死因子-α是影響類風濕關節(jié)炎患者血清網(wǎng)膜素水平的獨立相關因素,回歸方程為Y網(wǎng)膜素=16.416+0.751X腫瘤壞死因子-α。經(jīng)自擬加味四藤湯治療后類風濕關節(jié)炎患者血清網(wǎng)膜素降低程度與腫瘤壞死因子-α降低程度呈正相關(r=0.481,P=0.035)。結(jié)論:網(wǎng)膜素參與了類風濕關節(jié)炎的發(fā)病,并可能在類風濕關節(jié)炎的炎癥反應過程中對機體發(fā)揮一定的保護作用,其含量升高可能是其在炎癥狀態(tài)下的一種代償機制。
Abstract:
Objective:To explore the effects of serum omentin in the onset of rheumatoid arthritis(RA).Methods:Forty-eight RA patients were included into RA group and 45 healthy volunteers were included into control group as requested.Patients in the RA group were administrated with self-designed JIAWEI SITENG DECOCTION,while the others in the control group were administrated with nothing.The following indexes were measured as omentin,erythrocyte sedimentation rate(ESR),tumor necrosis factor alpha(TNF-α),interleukin 6(IL-6)and C reactive protein(CRP)in the blood collected from ulnar vein of patients in RA group with fasting for 14 hours before the treatment and 3 months after the treatment respectively,while the volunteers in the control group were processed with blood collection only once for measuring above mentioned indexes.After completion of collecting above mentioned indexes,the indexes were compared between RA group and control group before the treatment,and were compared between pretreatment and post-treatment for RA group,meanwhile,the relationship between serum omentin contents and other indexes of patients in RA group were analyzed.Results:The levels of TNF-α,IL-6,CRP,ESR and omentin of RA group were all higher than those of control group before the treatment(2.901±0.720 vs 0.815±0.411μg/L,t=-2.712,P=0.000; 8.043±3.706 vs 2.727±1.852ng/L,t=3.971,P=0.000; 19.384±7.450 vs 8.022±2.615mg/L,t=-10.562,P=0.000; 23.830±8.601 vs 12.734±5.910mm/h,t=-8.477,P=0.000; 30.525±3.970 vs 25.302±4.317μg/L,t=5.175,P=0.000).Contents of TNF-α,IL-6 and omentin of RA group were all lowered after the treatment(1.305±0.226μg/L,6.290±2.651ng/L,18.952±3.308mg/L,22.985±3.250mm/h,26.271±3.683μg/L)and there were statistical difference between pretreatment and post-treatment(t=1.905,P=0.000; t=4.991,P=0.000; t=3.226,P=0.005).There was no statistical difference in the levels of CRP and ESR between pretreatment and post-treatment(19.384±7.450 vs 18.952±3.308mg/L,t=2.157,P=0.113; 23.830±8.601 vs 22.985±3.250mm/h,t=3.401,P=0.098).The contents of Omentin were correlated positively with those of TNF-α and IL-6 in RA group(r=0.723,P=0.011; r=0.480,P=0.027).Multiple regression analysis was performed with taking the contents of serum omentin as dependent variable,while ESR,CRP,IL-6 and TNF-α as independent variables,as a result,TNF-α was found to be the independent correlative factor which affectted serum omentin levels in RA patients(Regression equation,YOM=16.416+0.751XTNF-α).After the treatment with self-designed JIAWEI SITENG DECOCTION,reduction of serum omentin was correlated positively with that of TNF-α for the RA patients(r=0.481,P=0.035).Conclusion:Omentin takes part in RA onset and it may partly protect the organism in the process of inflammatory reaction of RA,and the increase of its content may be a kind of compensation for the inflammation.
