摘要:
目的:探討手法整復(fù)、竹夾板外固定治療肱骨近端骨折的臨床療效。方法:將符合標(biāo)準(zhǔn)的68例肱骨近端骨折患者隨機(jī)分為2 組。夾板固定組35例,采用手法整復(fù)、竹夾板外固定治療; 石膏固定組33例,采用采用手法整復(fù)、石膏固定。比較2組患者的骨折 愈合時(shí)間及整復(fù)6周后的肩關(guān)節(jié)Neer評(píng)分。結(jié)果:石膏固定組有3例患者因疼痛對(duì)治療結(jié)果不滿意,2組其余患者均對(duì)治療結(jié)果滿意 ,均無(wú)明顯畸形。夾板固定組Neer評(píng)分高于石膏固定組,差異有統(tǒng)計(jì)學(xué)意義(t=9.357,P=0.000); 2組患者骨折愈合時(shí)間比較,差異 無(wú)統(tǒng)計(jì)學(xué)意義(臨床愈合時(shí)間:t=0.575,P=0.567; 骨性愈合時(shí)間:t=1.901,P=0.062)。結(jié)論:手法整復(fù)后采用竹夾板外固定能使患 者盡早開展肩關(guān)節(jié)功能鍛煉,有利于肱骨近端骨折患者肩關(guān)節(jié)功能恢復(fù)。
Abstract:
Objective:To explore the clinical curative effect of manipulative reduction and external fixation with bamboo splint in the treatment of proximal humerus fracture.Methods:68 patients with proximal humerus fracture,who met the criteria,were randomly divided into 2 groups.35 cases were administrated with manipulative reduction and external fixation with bamboo splint,while the others were administrated with manipulative reduction and plaster fixation.The fracture healing time were compared between the 2 groups,so did the Neer score of shoulder joint 6 weeks after reduction.Results:The patients in the 2 groups were all satisfied with the treatment result except 3 cases in plaster fixation group for the pain.No gross deformity was found in all the patients.Neer score of splintage group was higher than that of plaster fixation group and there was statistical difference between the 2 groups(t=9.357,P=0.000).There was no statistical difference in the fracture healing time between the 2 groups(clinical healing time:t=0.575,P=0.567; bone healing time:t=1.901,P=0.062).Conclusion:It is benefit for the patients with proximal humerus fracture to carry out earlier functional exercise of shoulder joint through the treatment with the external fixation with bamboo splint after the manipulative reduction,which is useful for the patients to recover the functions of their shoulder joint.
