摘要:
目的:系統(tǒng)評價止血帶在踝關(guān)節(jié)骨折切開復(fù)位內(nèi)固定術(shù)中的應(yīng)用價值。方法:應(yīng)用計算機檢索中國知網(wǎng)、萬方數(shù)據(jù)庫、維普網(wǎng)、中國生物醫(yī)學(xué)文獻服務(wù)系統(tǒng)、PubMed、Cochrane Library、Web of Science和Embase中有關(guān)踝關(guān)節(jié)骨折切開復(fù)位內(nèi)固定術(shù)中使用止血帶的隨機對照試驗文獻,檢索時限均為建庫至2024年4月。試驗組術(shù)中使用止血帶,對照組術(shù)中未使用止血帶。采用Cochrane系統(tǒng)評價手冊中的偏倚風險評估工具對納入研究的文獻進行質(zhì)量評價,采用RevMan4.3軟件進行Meta分析。結(jié)果:共檢索到53篇文獻,經(jīng)篩選后最終納入6篇文獻,涉及1347例患者,其中試驗組744例、對照組603例。Meta分析結(jié)果顯示,試驗組的手術(shù)時間短于對照組[MD=-6.80,95%CI(-12.14,-1.46),P=0.013],疼痛視覺模擬量表評分高于對照組[MD=1.01,95%CI(0.46,1.57),P=0.000],踝關(guān)節(jié)活動度小于對照組[MD=-5.25,95%CI(-9.61,-0.89),P=0.018],下肢深靜脈血栓發(fā)生率高于對照組[RR=6.34,95%CI(1.45,27.79),P=0.014],2組住院時間、切口感染發(fā)生率、切口延遲愈合或不愈合發(fā)生率的組間差異均無統(tǒng)計學(xué)意義[MD=1.88,95%CI(-0.48,4.23),P=0.118; RR=1.37,95%CI(0.97,1.94),P=0.074; RR=0.68,95%CI(0.39,1.19),P=0.174]。結(jié)論:現(xiàn)有的證據(jù)表明,在踝關(guān)節(jié)骨折切開復(fù)位內(nèi)固定術(shù)中使用止血帶可以縮短手術(shù)時間,但同時可能會導(dǎo)致下肢深靜脈血栓形成的風險增高、術(shù)后疼痛加重、踝關(guān)節(jié)活動范圍減小。
