摘要:
目的:探究胰島素樣生長(zhǎng)因子Ⅱ(insulin-like growth factorⅡ,IGF-Ⅱ)及胰島素樣生長(zhǎng)因子結(jié)合蛋白-6(insulin-like growth factor binding protein-6,IGFBP-6)血清水平與類風(fēng)濕關(guān)節(jié)炎(rheumatoid arthritis,RA)疾病活動(dòng)度的關(guān)系。方法:選取2019年2月至2020年12月收治的RA患者為研究對(duì)象,從病歷系統(tǒng)中提取性別、年齡、體質(zhì)量指數(shù)、病程、IGF-Ⅱ血清水平、IGFBP-6血清水平、28個(gè)關(guān)節(jié)疾病活動(dòng)性評(píng)分(disease activity score in 28 joints,DAS28)。IGF-Ⅱ血清水平、IGFBP-6血清水平、DAS28均選擇入院后24 h測(cè)定的結(jié)果。根據(jù)DAS28將患者分為高度活動(dòng)組(DAS28>5.1分)、中度活動(dòng)組(3.2分-1、(392.11±40.19)ng·mL-1、(472.39±39.76)ng·mL-1,3組患者的IGFBP-6血清水平分別為(502.67±39.86)ng·mL-1、(461.23±41.05)ng·mL-1、(386.51±43.17)ng·mL-1。輕度活動(dòng)組和中度活動(dòng)組的IGF-Ⅱ血清水平均低于高度活動(dòng)組(P=0.003,P=0.000)、IGFBP-6血清水平均高于高度活動(dòng)組(P=0.042,P=0.000); 輕度活動(dòng)組的IGF-Ⅱ血清水平低于中度活動(dòng)組(P=0.003),IGFBP-6血清水平高于中度活動(dòng)組(P=0.042)。Spearman相關(guān)分析結(jié)果顯示,IGF-Ⅱ血清水平與RA疾病活動(dòng)度呈正相關(guān)(rs=0.720,P=0.000),IGFBP-6血清水平與RA疾病活動(dòng)度呈負(fù)相關(guān)(rs=-0.671,P=0.001)。Logistic多因素回歸分析結(jié)果顯示,IGF-Ⅱ血清水平是RA疾病高度活動(dòng)的危險(xiǎn)因素[B=0.006,P=0.000,OR=1.007,95%CI(1.003,1.010)],IGFBP-6血清水平是RA疾病高度活動(dòng)的保護(hù)因素[B=-0.008,P=0.000,OR=0.992,95%CI(0.989,0.995)]。結(jié)論:IGF-Ⅱ血清水平與RA疾病活動(dòng)度呈正相關(guān),IGFBP-6血清水平與RA疾病活動(dòng)度呈負(fù)相關(guān),二者可在一定程度上反映RA疾病活動(dòng)度。
Abstract:
Objective:To explore the relationships between the serum levels of insulin-like growth factorⅡ(IGF-Ⅱ)and insulin-like growth factor binding protein-6(IGFBP-6)and the disease activity of rheumatoid arthritis(RA).Methods:The RA patients admitted from February 2019 to December 2020 were selected as the subjects,and the information including gender,age,body mass index,disease course,serum level of IGF-Ⅱ,serum level of IGFBP-6 and disease activity score in 28 joints(DAS28)was extracted from the Electronic Medical Record System(EMRS).The serum levels of IGF-Ⅱand IGFBP-6 and DAS28 measured at 24 hour after the hospital admission were selected.The patients were divided into high-activity group(DAS28>5.1 points),moderate-activity group(3.2 pointss=0.720,P=0.000),while a negative correlation between the serum level of IGFBP-6 and the disease activity of RA(rs=-0.671,P=0.001).The results of logistic multi-factor regression analysis showed that the increased serum level of IGF-Ⅱwas a risk factor for high disease activity in RA,while the increased serum level of IGFBP-6 acted as a protective factor against high disease activity in RA.Conclusion:The serum level of IGF-Ⅱis positively correlated with disease activity in RA,while the serum level of IGFBP-6 is negatively correlated with disease activity in RA,both of which can reflect the disease activity of RA to a certain extent.
