摘要:
目的:系統(tǒng)評(píng)價(jià)吸煙對(duì)開(kāi)放楔形脛骨高位截骨術(shù)(high tibial osteotomy,HTO)后截骨間隙骨愈合的影響。方法:應(yīng)用計(jì)算機(jī)檢索PubMed、Cochrane Library、Embase、中國(guó)知網(wǎng)、維普網(wǎng)和萬(wàn)方數(shù)據(jù)庫(kù)收錄的有關(guān)吸煙對(duì)開(kāi)放楔形HTO術(shù)后截骨間隙骨愈合影響的研究文獻(xiàn),檢索時(shí)限為數(shù)據(jù)庫(kù)建庫(kù)至2022年6月30日。由2名研究人員獨(dú)立按照納入和排除標(biāo)準(zhǔn)對(duì)相關(guān)文獻(xiàn)進(jìn)行篩選和數(shù)據(jù)提取,采用紐卡斯?fàn)?渥太華量表進(jìn)行文獻(xiàn)質(zhì)量評(píng)價(jià)后,采用RevMan5.4軟件進(jìn)行Meta分析,并采用漏斗圖進(jìn)行發(fā)表偏倚分析。結(jié)果:共檢索到35篇文獻(xiàn),經(jīng)過(guò)逐層篩選最終納入5篇文獻(xiàn),均為英文文獻(xiàn)。Meta分析結(jié)果顯示,吸煙組開(kāi)放楔形HTO術(shù)后截骨間隙骨不愈合率高于非吸煙組[RR=2.52,95%CI(1.72,3.67),P=0.000]。結(jié)論:吸煙會(huì)增加開(kāi)放楔形HTO術(shù)后截骨間隙骨不愈合的風(fēng)險(xiǎn)。
Abstract:
Objective:To systematically review the effects of smoking on bone healing of osteotomy gap caused by open-wedge high tibial osteotomy(OWHTO).Methods:All research articles about the effects of smoking on bone healing of osteotomy gap caused by OWHTO included from database establishing to June 30,2022 were retrieved from PubMed,Cochrane Library,Embase,China National Knowledge Internet,Vip Database and Wanfang Database through computer.The pertinent articles were screened and the information was extracted independently by two researchers according to the inclusion and exclusion criteria.If any disagreement was found between them,discussion was conducted or another researcher was invited for making a final decision.The methodological quality of researches in the articles was evaluated according to Newcastle-Ottawa scale(NOS),and then a Meta-analysis was conducted by using RevMan5.4 software.Furthermore,the publication bias was analyzed by using funnel plot.Results:Thirty-five articles were searched out.After screening,5 English articles were included in the final analysis.The results of Meta-analysis revealed that the bone nonunion rate of osteotomy gap caused by OWHTO was higher in smoking group compared to non-smoking group(RR=2.52,95%CI(1.72,3.67),P=0.000).Conclusion:Smoking can increase the risk of bone nonunion of osteotomy gap caused by OWHTO.
