摘要:
目的:系統(tǒng)評(píng)價(jià)中藥干預(yù)對(duì)骨質(zhì)疏松癥(osteoporosis,OP)大鼠骨密度及Wnt/β-聯(lián)蛋白(β-catenin)信號(hào)通路的影響。方法:應(yīng)用計(jì)算機(jī)檢索中國(guó)知網(wǎng)、維普網(wǎng)、萬方數(shù)據(jù)庫(kù)、中國(guó)生物醫(yī)學(xué)文獻(xiàn)服務(wù)系統(tǒng)、PubMed、Embase和Cochrane Library中關(guān)于中藥干預(yù)大鼠OP的文獻(xiàn),檢索時(shí)限均為建庫(kù)至2023年2月。實(shí)驗(yàn)組的干預(yù)措施為中藥(包括中藥復(fù)方、單味藥、中藥有效成分)灌胃,對(duì)照組的干預(yù)措施為生理鹽水或蒸餾水灌胃。依據(jù)文獻(xiàn)檢索及篩選方案篩選出符合要求的文獻(xiàn)后,由2名研究人員分別獨(dú)立進(jìn)行數(shù)據(jù)提取和質(zhì)量評(píng)價(jià)。采用Stata13.0軟件進(jìn)行Meta分析。結(jié)果:共檢索到210篇文獻(xiàn),最終納入30篇文獻(xiàn),共涉及1105只大鼠,其中實(shí)驗(yàn)組750只,對(duì)照組355只。Meta分析結(jié)果顯示,實(shí)驗(yàn)組大鼠股骨骨密度、腰椎骨密度及骨組織Wnt、β-catenin、Runt相關(guān)轉(zhuǎn)錄因子2、低密度脂蛋白受體相關(guān)蛋白5的mRNA和蛋白表達(dá)水平均高于對(duì)照組[SMD=1.75,95%CI(1.54,1.95),P=0.000; SMD=1.35,95%CI(1.07,1.64),P=0.000; SMD=3.82,95%CI(2.31,5.32),P=0.000; SMD=2.36,95%CI(2.00,2.72),P=0.000; SMD=3.07,95%CI(2.55,3.60),P=0.000; SMD=1.77,95%CI(1.40,2.14),P=0.000; SMD=5.11,95%CI(3.99,6.22),P=0.000; SMD=5.42,95%CI(4.04,6.81),P=0.000; SMD=1.74,95%CI(1.06,2.42),P=0.000; SMD=2.44,95%CI(1.47,3.41),P=0.000]。結(jié)論:中藥干預(yù)能夠提高OP大鼠的骨密度,其作用機(jī)制可能與其能夠上調(diào)Wnt/β-catenin信號(hào)通路相關(guān)蛋白的表達(dá)有關(guān)。
Abstract:
Objective:To systematically review the effects of traditional Chinese drugs(TCDs)intervention on bone mineral density(BMD)and Wnt/β-catenin signaling pathway in osteoporosis(OP)rats.Methods:All the articles about intervention on OP rats with TCDs included from database establishing to February 2023 were retrieved from China National Knowledge Internet,Vip Database,Wanfang Database,Chinese Biomedical Literature Service System,PubMed,Embase and Cochrane Library through computer.The rats in experiment group were intervened by intragastric administration with TCDs including compounds(TCD),simplex drug and Chinese medicine effective constituents,while the ones in control group with normal saline or distilled water.The articles were screened according to the retrieval and screening scheme.The information was extracted and the methodological quality of researches in the articles was evaluated independently by two researchers,and if any disagreement was found between them,discussion was conducted or another researcher was invited for making a final decision,and then a Meta-analysis was conducted by using Stata13.0 software.Results:Two hundred and ten articles were searched out.After screening,30 articles(1105 rats)were included in the final analysis,750 rats in experiment group and 355 ones in control group.The results of Meta-analysis revealed that the BMDs of femur and lumbar vertebrae as well as the mRNA and protein expression levels of Wnt,β-catenin,Runt-related transcription factor 2(RUNX2)and low-density lipoprotein receptor-related protein 5(LRP5)in bone tissues were higher in experiment group compared to control group(SMD=1.75,95%CI(1.54,1.95),P=0.000; SMD=1.35,95%CI(1.07,1.64),P=0.000; SMD=3.82,95%CI(2.31,5.32),P=0.000; SMD=2.36,95%CI(2.00,2.72),P=0.000; SMD=3.07,95%CI(2.55,3.60),P=0.000; SMD=1.77,95%CI(1.40,2.14),P=0.000; SMD=5.11,95%CI(3.99,6.22),P=0.000; SMD=5.42,95%CI(4.04,6.81),P=0.000; SMD=1.74,95%CI(1.06,2.42),P=0.000; SMD=2.44,95%CI(1.47,3.41),P=0.000).Conclusion:TCDs intervention can improve the BMD of OP rats,and its mechanism of action may be that it can up-regulate the expression of Wnt/β-catenin signaling pathway-related proteins in OP rats.
