摘要:
目的:系統(tǒng)評(píng)價(jià)國(guó)內(nèi)腰痛指南和共識(shí)的方法學(xué)質(zhì)量。方法:計(jì)算機(jī)檢索中國(guó)知網(wǎng)、萬(wàn)方數(shù)據(jù)庫(kù)、維普網(wǎng)、醫(yī)脈通網(wǎng)、PubMed和Embase,搜集國(guó)內(nèi)已發(fā)布的腰痛指南和共識(shí),檢索時(shí)限均為2016年1月至2021年10月。經(jīng)過(guò)文獻(xiàn)篩選、數(shù)據(jù)提取后,由3名研究者獨(dú)立采用臨床指南研究與評(píng)價(jià)系統(tǒng)Ⅱ(appraisal of guidelines for research and evaluationⅡ,AGREEⅡ)對(duì)納入指南和共識(shí)的方法學(xué)質(zhì)量進(jìn)行評(píng)價(jià)。采用組內(nèi)相關(guān)系數(shù)(interclass correlation coefficient,ICC)檢驗(yàn)3名研究者評(píng)分的一致性。結(jié)果:共納入19篇腰痛指南和共識(shí),在AGREEⅡ6個(gè)領(lǐng)域的評(píng)分分別為范圍與目的(64.04±17.99)%、參與人員(25.73±14.61)%、制定嚴(yán)謹(jǐn)性(25.88±20.55)%、表達(dá)清晰性(49.42±16.45)%、應(yīng)用性(15.13±7.76)%、編輯獨(dú)立性(1.32±3.12)%。3名研究者評(píng)價(jià)的總體一致性較好[ICC=0.87,95%CI(0.76,0.95)]。結(jié)論:國(guó)內(nèi)現(xiàn)有腰痛指南和共識(shí)的方法學(xué)質(zhì)量總體不高; 未來(lái)的相關(guān)指南需要在方法學(xué)質(zhì)量方面進(jìn)一步提高,尤其是在參與人員、制定嚴(yán)謹(jǐn)性、應(yīng)用性和編輯獨(dú)立性方面。
Abstract:
Objective:To systematically evaluate the methodological quality of guidelines and consensuses in China on low back pain.Methods:The published guidelines and consensuses on low back pain were retrieved from China National Knowledge Infrastructure(CNKI),Wanfang Data,VIP,Medlive,PubMed,and Embase from January 2016 to October 2021.After literature screening and data extraction,three investigators independently evaluated the methodological quality of the included guidelines and consensuses by virtue of the Appraisal of Guidelines for Research and EvaluationⅡ(AGREEⅡ).The consistency of the scores of the three investigators was examined by the interclass correlation coefficient(ICC).Results:Nineteen guidelines and consensus on low back pain were included.The scores for six domains of AGREEⅡare listed below—(64.04±17.99)% for scope and purpose,(25.73±14.61)% for stakeholder involvement,(25.88±20.55)% for the rigor of development,(49.42±16.45)% for clarity of presentation,(15.13±7.76)% for applicability,and(1.32±3.12)% for editorial independence.The overall consistency of the three investigators was good(ICC=0.87,95%CI(0.76,0.95)).Conclusion:The methodological quality of the guidelines and consensus available in China on low back pain is not high.Guidelines need to be further improved in methodological quality,particularly in terms of stakeholder involvement,rigor of development,applicability,and editorial independence.
