摘要:
目的:觀察踝關(guān)節(jié)鏡下清理聯(lián)合局部微骨折術(shù)治療距骨骨樣骨瘤的臨床療效和安全性。方法:2018年4月至2020年12月,采用踝關(guān)節(jié)鏡下清理聯(lián)合局部微骨折術(shù)治療距骨骨樣骨瘤患者7例。男5例,女2例。年齡17~23歲,中位數(shù)20歲。病變位于左側(cè)距骨頸3例,右側(cè)距骨頸3例,左側(cè)距骨體1例。分別于術(shù)前、術(shù)后3個(gè)月,采用踝部疼痛視覺(jué)模擬量表(visual analogue scale,VAS)評(píng)價(jià)踝關(guān)節(jié)疼痛情況,采用美國(guó)足與踝關(guān)節(jié)協(xié)會(huì)(American Orthopaedic Foot and Ankle Society,AOFAS)踝與后足評(píng)分標(biāo)準(zhǔn)評(píng)價(jià)踝關(guān)節(jié)功能。隨訪觀察并發(fā)癥發(fā)生情況。結(jié)果:本組7例,手術(shù)均順利完成。所有患者均獲隨訪,隨訪時(shí)間8~12個(gè)月,中位數(shù)10個(gè)月。踝關(guān)節(jié)疼痛VAS評(píng)分,術(shù)前(6.43±1.27)分,術(shù)后3個(gè)月(0.43±0.53)分; AOFAS踝與后足評(píng)分,術(shù)前(39.57±10.44)分,術(shù)后3個(gè)月(92.86±3.58)分。均未發(fā)生切口感染、腫瘤復(fù)發(fā)等并發(fā)癥。結(jié)論:采用踝關(guān)節(jié)鏡下清理聯(lián)合局部微骨折術(shù)治療距骨骨樣骨瘤,能夠緩解踝關(guān)節(jié)疼痛,改善踝關(guān)節(jié)功能,且安全性高。
