摘要:
目的:探討Ramp損傷的MRI表現(xiàn)特征。方法:回顧性分析21例前交叉韌帶(anterior cruciate ligament,ACL)斷裂合并Ramp損傷患者的病例資料。男16例,女5例。年齡13~51歲,中位數(shù)28歲。由2名高年資影像專業(yè)醫(yī)師分析其MRI資料,采用Greif等提出的Ramp損傷分型標(biāo)準(zhǔn)劃分Ramp損傷類型,總結(jié)Ramp損傷的MRI表現(xiàn)特征。結(jié)果:本組21例,Ramp損傷3B型2例、4A型13例、5型6例。脂肪抑制質(zhì)子密度加權(quán)像(fat suppression-proton density weighted imaging,FS-PDWI)上21例患者的ACL均呈明顯高信號,纖維束部分或全部斷裂。19例矢狀位FS-PDWI上內(nèi)側(cè)半月板后角紅區(qū)形態(tài)不規(guī)則,呈垂直或斜形高信號并累及關(guān)節(jié)面; 2例矢狀位FS-PDWI上半月板脛骨韌帶內(nèi)側(cè)半月板后角附著處不連續(xù),呈高信號填充,殘端韌帶走行扭曲。4例矢狀位及橫斷位FS-PDWI顯示脛股關(guān)節(jié)不穩(wěn),脛骨相對股骨前移、旋轉(zhuǎn); 12例矢狀位、冠狀位及橫斷位FS-PDWI上脛骨內(nèi)側(cè)平臺后緣呈斑片狀高信號,提示脛骨內(nèi)側(cè)平臺后緣骨挫傷。結(jié)論:Ramp損傷的MRI表現(xiàn)特征多為FS-PDWI上內(nèi)側(cè)半月板后角紅區(qū)形態(tài)不規(guī)則,呈垂直或斜形高信號并累及關(guān)節(jié)面; 或半月板脛骨韌帶內(nèi)側(cè)半月板后角附著處不連續(xù),呈高信號填充; 部分脛骨相對股骨前移旋轉(zhuǎn),脛骨內(nèi)側(cè)平臺后緣呈斑片狀高信號。
