摘要:
目的:評(píng)價(jià)髓芯減壓植骨聯(lián)合應(yīng)用富血小板血漿(platelet-rich plasma,PRP)治療股骨頭壞死的有效性。方法:應(yīng)用計(jì)算機(jī)檢索PubMed、Embase、Cochrane Library、中國(guó)知網(wǎng)、萬(wàn)方數(shù)據(jù)庫(kù)、維普網(wǎng),搜集應(yīng)用髓芯減壓植骨聯(lián)合應(yīng)用PRP治療股骨頭壞死的隨機(jī)對(duì)照試驗(yàn)文獻(xiàn),試驗(yàn)組采用髓芯減壓植骨聯(lián)合應(yīng)用PRP治療、對(duì)照組采用髓芯減壓植骨治療。檢索時(shí)限均為數(shù)據(jù)庫(kù)建立至2020年12月。經(jīng)文獻(xiàn)檢索、文獻(xiàn)篩選、數(shù)據(jù)提取及偏倚風(fēng)險(xiǎn)評(píng)價(jià)后,采用RevMan5.3軟件對(duì)術(shù)后髖關(guān)節(jié)疼痛視覺模擬量表(visual analogue scale,VAS)評(píng)分、術(shù)后Harris髖關(guān)節(jié)評(píng)分、影像學(xué)改善率及人工髖關(guān)節(jié)置換率進(jìn)行Meta分析。結(jié)果:共納入13項(xiàng)隨機(jī)對(duì)照試驗(yàn)。Meta分析結(jié)果顯示,試驗(yàn)組患者的術(shù)后髖關(guān)節(jié)疼痛VAS評(píng)分低于對(duì)照組[I2=83%,MD=-0.81,95%CI(-1.05,-0.57),P=0.000],術(shù)后Harris髖關(guān)節(jié)評(píng)分高于對(duì)照組[I2=87%,MD=7.88,95%CI(6.00,9.76),P=0.000],影像學(xué)改善率高于對(duì)照組[I2=4%,OR=5.12,95%CI(2.90,9.02),P=0.000],人工髖關(guān)節(jié)置換率低于對(duì)照組[I2=0%,OR=0.32,95%CI(0.12,0.82),P=0.020]。結(jié)論:現(xiàn)有的證據(jù)表明,髓芯減壓植骨聯(lián)合應(yīng)用PRP治療股骨頭壞死的療效優(yōu)于單純的髓芯減壓植骨治療。
Abstract:
Objective:To evaluate the clinical efficacy of core decompression and bone grafting combined with application of platelet-rich plasma(PRP)in treatment of osteonecrosis of femoral head(ONFH).Methods:All the randomized controlled trial(RCT)articles about core decompression and bone grafting combined with application of PRP for treatment of ONFH included from database establishing to December 2020 were retrieved from the PubMed,Embase,Cochrane Library,China National Knowledge Internet,Wanfang Database and Vip Database through computer.The patients in experimental group were treated with core decompression and bone grafting combined with PRP,while the ones in control group with core decompression and bone grafting alone.The articles were retrieved and screened and the information was extracted independently by two researchers according to the inclusion and exclusion criteria.The bias risk of the research in the articles was evaluated by using RCT bias risk assessment tools recommended by Cochrane systematic evaluation handbook,and then a Meta-analysis on postoperative hip pain visual analogue scale(VAS)score,postoperative Harris hip score,imaging improvement rate and artificial hip replacement rate was conducted by using RevMan5.3 software.Results:Thirteen articles were included in the final analysis.The results of Meta-analysis revealed that the postoperative hip pain VAS score and artificial hip replacement rate were lower,while the postoperative Harris hip score and imaging improvement rate were higher in experimental group compared to control group(I2=83%,MD=-0.81,95%CI(-1.05,-0.57),P=0.000; I2=0%,OR=0.32,95%CI(0.12,0.82),P=0.020; I2=87%,MD=7.88,95%CI(6.00,9.76),P=0.000; I2=4%,OR=5.12,95%CI(2.90,9.02),P=0.000).Conclusion:Available evidences suggest that core decompression and bone grafting combined with application of PRP has better clinical efficacy compared to core decompression and bone grafting alone in treatment of ONFH.
