摘要:
目的:探討頸椎曲度與脊髓型頸椎病的關(guān)系。方法:選取2018年1月至2020年1月在鄭州大學(xué)第一附屬醫(yī)院接受頸椎X線檢查的114例脊髓型頸椎病患者(脊髓型頸椎病組)和同期在醫(yī)院進(jìn)行頸椎X線檢查的61例健康體檢者(非頸椎病組)進(jìn)行研究。采用Borden法在拍攝的側(cè)位X線片上測量頸椎曲線深度(cervical curve depth,CCD),比較2組受試者的CCD和頸椎曲度異常率,采用受試者操作特征(receiver operating characteristic,ROC)曲線分析評價(jià)CCD診斷脊髓型頸椎病的價(jià)值。結(jié)果:脊髓型頸椎病組的CCD小于非頸椎病組[(3.14±5.51)mm,(8.87±2.43)mm,t=-7.715,P=0.000]; 脊髓型頸椎病組頸椎曲度正常12例、異常102例,非頸椎病組頸椎曲度正常56例、異常5例,脊髓型頸椎病組的頸椎曲度異常率高于非頸椎病組(χ2=110.488,P=0.000)。ROC曲線分析結(jié)果顯示,依據(jù)CCD診斷脊髓型頸椎病的靈敏度為83.3%、特異度為90.2%、約登指數(shù)為74.4%、曲線下面積為0.876、診斷界值為6.945 mm。結(jié)論:脊髓型頸椎病的發(fā)生與頸椎曲度減小有關(guān),CCD小于6.945 mm時(shí)發(fā)生脊髓型頸椎病的可能性較大。
Abstract:
Objective:To explore the relationship between cervical spine curvature(CSC)and cervical spondylosis(CS)myelopathy(CSM).Methods:One hundred and fourteen CSM patients(CSM group)and 61 healthy volunteers(non-CS group)who received cervical X-ray examination in The First Affiliated Hospital of Zhengzhou University from January 2018 to January 2020 were selected for the study.The cervical curve depth(CCD)were measured on the lateral X-ray films by using Borden’s method.The CCD and CSC abnormal rate were compared between the 2 groups,and the value of CCD in diagnosing CSM was analyzed and evaluated by using receiver operating characteristic(ROC)curve.Results:The CCD was smaller in CSM group compared to non-CS group(3.14±5.51 vs 8.87±2.43 mm,t=-7.715,P=0.000).The normal CSC was found in 12 patients in CSM group and 56 ones in non-CS group,and the abnormal CSC was found 102 patients in CSM group and 5 ones in non-CS group.The CSC abnormal rate was higher in CSM group compared to non-CS group(χ2=110.488,P=0.000).The results of ROC curve analysis showed that the sensitivity,specificity,Yoden index,area under curve and diagnostic cut-off value of CCD in diagnosing CSM were 83.3%,90.2%,74.4%,0.876 and 6.945 mm respectively.Conclusion:The onset of CSM is related to the decrease of CSC and it is a great possibility when the CCD is less than 6.945 mm.
