摘要:
目的:分析肩胛骨岡盂切跡的形態(tài)學(xué)分型特征。方法:納入460例接受胸部或肩部CT檢查的患者,將其CT掃描數(shù)據(jù)傳輸至西門子影像存儲(chǔ)與傳輸系統(tǒng)中進(jìn)行單側(cè)肩胛骨三維重建,并在CT三維重建圖像上測量和記錄肩胛骨岡盂切跡的形態(tài)學(xué)數(shù)據(jù); 根據(jù)岡盂切跡的形態(tài)將460例患者分別納入L形組、大U形組和小U形組,分析比較3組肩胛骨岡盂切跡的形態(tài)學(xué)特征。結(jié)果:L形岡盂切跡134例(L形組)、大U形103例(大U形組)、小U形223例(小U形組)。3組患者岡盂切跡至肩胛切跡、盂上結(jié)節(jié)、盂下結(jié)節(jié)的距離及岡盂切跡的深度、寬度比較,組間差異均有統(tǒng)計(jì)學(xué)意義(F=20.911,P=0.028; F=15.095,P=0.017; F=17.095,P=0.001; F=12.932,P=0.001; F=9.374,P=0.012); 小U形組岡盂切跡至肩胛切跡、盂上結(jié)節(jié)、盂下結(jié)節(jié)的距離及岡盂切跡的深度、寬度均小于L形組和大U形組(岡盂切跡至肩胛切跡的距離:q=1.928,P=0.029; q=2.018,P=0.030; 岡盂切跡至盂上結(jié)節(jié)的距離:q=12.012,P=0.001; q=9.253,P=0.001; 岡盂切跡至盂下結(jié)節(jié)的距離:q=9.267,P=0.001; q=5.267,P=0.001; 岡盂切跡的深度:q=3.191,P=0.001; q=4.353,P=0.001; 岡盂切跡的寬度:q=2.803,P=0.003; q=3.656,P=0.003); L形組岡盂切跡至肩胛切跡、盂上結(jié)節(jié)、盂下結(jié)節(jié)的距離及岡盂切跡的深度、寬度與大U形組比較,差異無統(tǒng)計(jì)學(xué)意義(q=2.803,P=0.146; q=3.128,P=0.149; q=3.412,P=0.111; q=8.453,P=1.646; q=3.780,P=0.852)。3組患者岡盂切跡至肩胛上角、肩胛下角、關(guān)節(jié)盂中央的距離及α角(以岡盂切跡頂點(diǎn)為頂點(diǎn)、以肩胛骨橫軸和肩胛岡長軸為邊所構(gòu)成的角)比較,組間差異均無統(tǒng)計(jì)學(xué)意義(F=5.546,P=0.129; F=8.977,P=1.092; F=6.029,P=0.941; F=2.091,P=0.589)。結(jié)論:與L形和大U形岡盂切跡相比,小U形岡盂切跡至肩胛切跡、盂上結(jié)節(jié)、盂下結(jié)節(jié)的距離及小U形岡盂切跡的深度、寬度均小。
Abstract:
Objective:To analyze the morphological classification characteristics of spinoglenoid notch(SGN).Methods:Four hundred sixty patients who underwent chest or shoulder computed tomography(CT)examination were enrolled in the study,and their CT data were transmitted to the Siemens Picture Archiving and Communication System(PACS)for three-dimensional(3D)reconstruction of unilateral scapula.According to the morphological data of SGN measured on the 3D CT images,the 460 patients were grouped into the L-shaped group,large U-shaped group and small U-shaped group,and the morphological characteristics of SGN were analyzed and compared among the 3 groups.Results:The L-shaped SGN,large U-shaped SGN and small U-shaped SGN were found in 134 cases,103 cases and 223 cases in L-shaped group,large U-shaped group and small U-shaped group respectively.The 3 groups showed statistically significant differences in the distances from the SGN to the scapular notch(ab),supraglenoid tubercle(ae)and infraglenoid tubercle(af)as well as the depth(S1)and width(S2)of SGN(F=20.911,P=0.028; F=15.095,P=0.017; F=17.095,P=0.001; F=12.932,P=0.001; F=9.374,P=0.012.The values of ab,ae,af,S1 and S2 were smaller in small U-shaped group compared to L-shaped group and large U-shaped group(ab:q=1.928,P=0.029; q=2.018,P=0.030; ae:q=12.012,P=0.001; q=9.253,P=0.001; af:q=9.267,P=0.001; q=5.267,P=0.001; S1:q=3.191,P=0.001; q=4.353,P=0.001; S2:q=2.803,P=0.003; q=3.656,P=0.003),while there was no significant difference between L-shaped group and large U-shaped group(q=2.803,P=0.146; q=3.128,P=0.149; q=3.412,P=0.111; q=8.453,P=1.646; q=3.780,P=0.852).There was no statistical difference among the 3 groups in the distances from SGN to superior angle of scapula(ac),subscapular angle(ad)and the middle of glenoid cavity(ag)as well as the angle α between the lateral axis of the scapula and the long axis of scapular spine when the vertex was at the nadir of the SGN(F=5.546,P=0.129; F=8.977,P=1.092; F=6.029,P=0.941; F=2.091,P=0.589).Conclusion:The small U-shaped SGN has smaller ab,ae,af,S1 and S2 compared to the L-shaped SGN and large U-shaped SGN.
