摘要:
脊髓型頸椎病(cervical spondylotic myelopathy,CSM)一旦確診,經(jīng)規(guī)范非手術(shù)治療無(wú)效或神經(jīng)功能障礙進(jìn)行性加重,應(yīng)盡早手術(shù)治療,但在具體術(shù)式選擇方面尚存爭(zhēng)議。傳統(tǒng)前路頸椎間盤切除融合術(shù)(anterior cervical discetomy and fusion,ACDF)是治療CSM的金標(biāo)準(zhǔn); 但融合術(shù)后手術(shù)節(jié)段運(yùn)動(dòng)功能喪失,致其相鄰節(jié)段應(yīng)力增加,存在加速相鄰節(jié)段退變(adjacent segment degeneration,ASD)的可能。頸椎人工椎間盤置換術(shù)(artificial cervical disc replacement,ACDR)可維持病變節(jié)段活動(dòng)度,理論上可減少ASD的發(fā)生。對(duì)于CSM前路手術(shù)中的融合與非融合問(wèn)題,有必要進(jìn)行深入探討,以利于手術(shù)方式的合理選擇。本文分別對(duì)ACDF、頸椎前路椎體次全切除融合術(shù)、ACDR和前路“雜交式”減壓術(shù)的優(yōu)缺點(diǎn)進(jìn)行了總結(jié)和評(píng)述。
