摘要:
Lisfranc損傷是一種嚴(yán)重的中足損傷,近年來其發(fā)生率呈上升趨勢。但由于解剖結(jié)構(gòu)和損傷機制復(fù)雜,依據(jù)X線片進(jìn)行診斷,大約20%的Lisfranc損傷會被漏診或誤診。因此,對于疑似Lisfranc損傷的患者,依據(jù)常規(guī)X線片診斷不明確時,應(yīng)拍攝負(fù)重位X線片或進(jìn)行CT、MRI檢查以明確診斷。Lisfranc損傷的治療包括非手術(shù)治療和手術(shù)治療2大類。手術(shù)治療Lisfranc損傷的方法主要有切開復(fù)位內(nèi)固定術(shù)和一期關(guān)節(jié)融合固定術(shù)。Lisfranc損傷常用的內(nèi)固定方法包括螺釘固定、微型鋼板固定、克氏針固定等,但哪種內(nèi)固定方法才是治療Lisfranc損傷的最佳方法仍存在相當(dāng)大的爭議。為進(jìn)一步提高臨床醫(yī)生對Lisfranc損傷的認(rèn)識,本文從Lisfranc關(guān)節(jié)的解剖特點和損傷機制、Lisfranc損傷的診斷和分型、Lisfranc損傷的治療3個方面對近年來Lisfranc損傷的診治進(jìn)展進(jìn)行了綜述。
