摘要:
目的:探討用于治療骨折延遲愈合和骨折不愈合的中藥湯劑的用藥規(guī)律。方法:應(yīng)用計(jì)算機(jī)檢索中國知網(wǎng)、萬方數(shù)據(jù)、維普網(wǎng)及中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫建庫至2018年6月1日收錄的所有口服中藥湯劑治療骨折延遲愈合和骨折不愈合的文獻(xiàn)。通過閱讀題目、摘要及全文篩選文獻(xiàn),提取文獻(xiàn)中涉及的方劑及藥物,分析用藥規(guī)律。結(jié)果:最初共檢索到2203篇文獻(xiàn),通過篩選后納入110篇文獻(xiàn),涉及方劑115首、藥物57種,57種藥物共出現(xiàn)1151次。57種藥物中,出現(xiàn)頻次≥20次的藥物有22種,按照出現(xiàn)頻次從高至低排在前10位的依次為當(dāng)歸(79次)、骨碎補(bǔ)(65次)、熟地黃(61次)、續(xù)斷(56次)、甘草(51次)、自然銅(46次)、黃芪(45次)、丹參(41次)、牛膝(40次)、川芎(38次)。57種藥物涉及的藥性包括溫、平、寒、熱,以溫(639次)、平(329次)、寒(177次)為主; 酸、苦、甘、辛、咸、淡、澀7種藥味均有涉及,以甘(788次)、苦(512次)、辛(441次)味藥為主; 歸各臟腑經(jīng)絡(luò)的藥物均有,以歸肝經(jīng)(860次)、腎經(jīng)(509次)、脾經(jīng)(454次)、心經(jīng)(343次)、肺經(jīng)(207次)、胃經(jīng)(141次)藥物為主; 57種藥物中具有活血化瘀(452次)、和營止痛(285次)、接骨續(xù)筋(247次)、補(bǔ)益肝腎(242次)、補(bǔ)氣養(yǎng)血(146次)、補(bǔ)脾養(yǎng)胃(125次)作用的藥物出現(xiàn)的頻次較多。結(jié)論:用于骨折延遲愈合和骨折不愈合的中藥湯劑中的高頻中藥包括當(dāng)歸、骨碎補(bǔ)、熟地黃、續(xù)斷、甘草、自然銅、黃芪、丹參、牛膝、川芎,藥性以溫、平、寒為主,藥味以甘、苦、辛為主,歸經(jīng)以歸肝經(jīng)、腎經(jīng)、脾經(jīng)、心經(jīng)、肺經(jīng)、胃經(jīng)為主,功效以活血化瘀、和營止痛、接骨續(xù)筋、補(bǔ)益肝腎、補(bǔ)氣養(yǎng)血、補(bǔ)脾養(yǎng)胃為主。
Abstract:
Objective:To explore the medication rule of Traditional Chinese Medicine(TCM)decoctions for treatment of delayed union and nonunion of fractures.Methods:All articles about oral application of TCM decoctions for treatment of delayed union and nonunion of fractures included from database establishing to June 1,2018 were retrieved from China national knowledge internet,WanFang Data,VIP Database and Chinese biomedical literature database through computer.The articles were screened through reading titles,abstracts and full texts,and the prescriptions and drugs involved in the articles were searched out,and their medication rules were analyzed.Results:Two thousand two hundred and three articles were searched out.After screening,110 articles were included in the final analysis,including 115 prescriptions and 57 drugs(total frequency was 1151).In 57 drugs,the occurrence frequency of 22 drugs were ≥ 20,and the top 10 drugs with high occurrence frequency included Chinese angelica(79 times),drynaria fortunei(65 times),radix rehmanniae praeparata(61 times),dipsacus asper(56 times),radix glycyrrhiza(51 times),pyritum(46 times),astragalus membranaceus(45 times),salvia miltiorrhiza(41 times),achyranthes bidentata(40 times)and ligusticum chuanxiong(38 times)in turn.For the 57 drugs,the drug properties included warm,normal,cold and hot and presented mainly with warm(639 times),normal(329 times)and cold(177 times); the drug flavours included sour,bitter,sweet,pungent,salty,bland and astringency and presented mainly with sweet(788 times),bitter(512 times)and pungent(441 times); the meridian distributions included all entrails meridians and presented mainly with liver meridian(860 times),kidney meridian(509 times),spleen meridian(454 times),heart meridiam(343 times),lung meridian(207 times)and stomach meridian(141 times); the clinical effects presented mainly with activating blood and resolving stasis(452 times),harmonizing nutrient and relieving pain(285 times),reuniting ruptured sinews and bones(247 times),tonifying liver and kidney(242 times),tonifying qi and nourishing blood(146 times)and tonifying spleen and nourishing stomach(125 times).Conclusion:The Chinese drugs with high occurrence frequency in TCM decoctions which are used for treatment of delayed union and nonunion of fractures include Chinese angelica,drynaria fortunei,radix rehmanniae praeparata,dipsacus asper,radix glycyrrhiza,pyritum,astragalus membranaceus,salvia miltiorrhiza,achyranthes bidentata and ligusticum chuanxiong.Their drug properties present mainly with warm,normal and cold; and their drug flavours present mainly with sweet,bitter and pungent.Their meridian distributions present mainly with liver meridian,kidney meridian,spleen meridian,heart meridiam,lung meridian and stomach meridian; and their clinical effects present mainly with activating blood and resolving stasis,harmonizing nutrient and relieving pain,reuniting ruptured sinews and bones,tonifying liver and kidney,tonifying qi and nourishing blood and tonifying spleen and nourishing stomach.
