摘要:
目的:觀察放散式體外沖擊波聯(lián)合關(guān)節(jié)腔注射和功能鍛煉治療肩周炎的臨床療效。方法:將126例肩周炎患者隨機(jī)分為2組,63例采用放散式體外沖擊波聯(lián)合關(guān)節(jié)腔注射和功能鍛煉治療(聯(lián)合治療組),63例采用關(guān)節(jié)腔注射聯(lián)合功能鍛煉治療(關(guān)節(jié)腔注射組)。放散式體外沖擊波治療,2周1次,共治療4次; 關(guān)節(jié)腔注射采用玻璃酸鈉注射液和曲安奈德注射液,每周1次,共治療4次; 功能鍛煉,每天3～5次,共治療30 d。分別采用視覺模擬量表(visual analogue scale,VAS)和肩關(guān)節(jié)評(píng)價(jià)量表評(píng)定患者的肩關(guān)節(jié)疼痛程度和總體療效。結(jié)果:治療前2組患者的肩關(guān)節(jié)疼痛VAS評(píng)分比較,差異無統(tǒng)計(jì)學(xué)意義(t=1.761,P=0.953); 治療結(jié)束后2組患者的VAS評(píng)分均減小[(7.08±2.04)分,(2.62±0.92)分,t=3.726,P=0.021;(7.15±1.87)分,(3.15±1.02)分,t=2.854,P=0.031],聯(lián)合治療組的VAS評(píng)分低于關(guān)節(jié)腔注射組(t=2.794,P=0.031)。治療前2組患者的肩關(guān)節(jié)綜合評(píng)分比較,差異無統(tǒng)計(jì)學(xué)意義(t=1.642,P=0.735); 治療結(jié)束后2組患者的肩關(guān)節(jié)綜合評(píng)分均增大[(45.80±10.52)分,(87.27±4.54)分,t=3.826,P=0.023;(46.95±11.09)分,(73.63±3.86)分,t=2.971,P=0.034],聯(lián)合治療組的肩關(guān)節(jié)綜合評(píng)分高于關(guān)節(jié)腔注射組(t=2.014,P=0.039)。結(jié)論:采用放散式體外沖擊波聯(lián)合關(guān)節(jié)腔注射和功能鍛煉治療肩周炎療效確切,效果優(yōu)于關(guān)節(jié)腔注射聯(lián)合功能鍛煉。
Abstract:
ABSTRACT Objective:To observe the clinical curative effect of divergent type extracorporeal shock wave(ESW)therapy combined with intra-articular injection and functional exercises in the treatment of periarthritis humeroscapularis.Methods:One hundred and twenty-six patients with periarthritis humeroscapularis were randomly divided into 2 groups,63 cases in each group.The patients were treated with combination therapy of divergent type ESW therapy and intra-articular injection and functional exercises(combination therapy group)and combination therapy of intra-articular injection and functional exercises(intra-articular injection group)respectively.The divergent type ESW therapy was performed once 2 weeks for consecutive 4 times,and the intra-articular injection of sodium hyaluronate and triamcinolone acetonide was performed once a week for consecutive 4 times,and the functional exercises was performed 3-5 times a day for consecutive 30 days.The shoulder pain degree and the total clinical curative effects were evaluated by using visual analogue scale(VAS)and shoulder rating scale respectively.Results:There was no statistical differences in shoulder pain VAS scores between the 2 groups before the treatment(t=1.761,P=0.953).The VAS scores decreased in both of the 2 groups after the end of the treatment(7.08+/-2.04 vs 2.62+/-0.92 points,t=3.726,P=0.021; 7.15+/-1.87 vs 3.15+/-1.02 points,t=2.854,P=0.031),and the VAS scores were lower in combination therapy group compared to intra-articular injection group(t=2.794,P=0.031).There was no statistical differences in shoulder comprehensive scores between the 2 groups before the treatment(t=1.642,P=0.735).The shoulder comprehensive scores increased in both of the 2 groups after the end of the treatment(45.80+/-10.52 vs 87.27+/-4.54 points,t=3.826,P=0.023; 46.95+/-11.09 vs 73.63+/-3.86 points,t=2.971,P=0.034),and the shoulder comprehensive scores were higher in combination therapy group compared to intra-articular injection group(t=2.014,P=0.039).Conclusion:The curative effect of combination therapy of ESW therapy and intra-articular injection and functional exercises is reliable for treatment of periarthritis humeroscapularis,and it is better than that of intra-articular injection combined with functional exercises.
