摘要:
目的:初步探討膝關(guān)節(jié)關(guān)節(jié)液和血清中骨鈣素(bone gla protein,BGP)、基質(zhì)金屬蛋白酶1(matrix metalloproteinase 1,MMP-1)及胰島素樣生長(zhǎng)因子Ⅰ(insulin-like growth factorⅠ,IGF-Ⅰ)含量與軟骨損傷的關(guān)系。方法:納入80例患者,其中 因膝骨關(guān)節(jié)炎(knee osteoarthritis,KOA)接受膝關(guān)節(jié)置換或膝關(guān)節(jié)鏡治療,直視或鏡下確認(rèn)關(guān)節(jié)軟骨損傷的患者40例(KOA組); 因膝關(guān)節(jié)游離體或陳舊性交叉韌帶損傷或單純半月板損傷行膝關(guān)節(jié)鏡手術(shù),經(jīng)關(guān)節(jié)鏡檢查關(guān)節(jié)軟骨無(wú)明顯損傷的患者40例(對(duì)照 組)。采用ELISA法測(cè)定患者關(guān)節(jié)液和血清中BGP、MMP-1及IGF-Ⅰ的含量,并進(jìn)行組間對(duì)比。結(jié)果:KOA組患者膝關(guān)節(jié)液和血清中 的BGP、MMP-1及IGF-Ⅰ含量均高于對(duì)照組[BGP:(17.91±2.51)μg·L-1,(13.43±1.20)μg·L- 1,t=19.735,P=0.000;(7.04±2.03)μg·L-1,(6.08±1.96)μg·L- 1,t=9.387,P=0.000; MMP-1:(13.79±2.51)μg·L-1,(12.96±2.39)μg·L- 1,t=10.979,P=0.000;(8.01±2.01)μg·L-1,(6.79±1.57)μg·L- 1,t=17.403,P=0.000; IGF-Ⅰ:(15.63±3.51)μg·L-1,(11.63±2.87)μg·L- 1,t=40.970,P=0.000;(6.09±2.39)μg·L-1,(4.30±2.01)μg·L- 1,t=20.340,P=0.000]。結(jié)論:測(cè)定膝關(guān)節(jié)關(guān)節(jié)液和血清中的BGP、MMP-1、IGF-Ⅰ含量,可作為診斷膝關(guān)節(jié)軟骨損傷的輔 助方法之一。
Abstract:
Objective:To explore the relationship between knee cartilage injury and contents of bone gla protein (BGP),matrix metalloproteinase 1(MMP-1)and insulin-like growth factorⅠ(IGF-Ⅰ)in knee synovial fluid and serum.Methods:Eighty patients were enrolled in the study,40 of which were treated with knee replacement or arthroscopy for knee osteoarthritis(KOA)and the articular cartilage injuries were confirmed in them(KOA group),while other patients were treated with knee arthroscopic surgery for knee liber bodies or old cruciate ligament injury or simple meniscus injury and no obvious knee cartilage injuries were found in them(control group).The contents of BGP,MMP-1 and IGF-Ⅰin knee synovial fluid and serum were measured by using ELISA and compared between the 2 groups.Results:The contents of BGP,MMP-1 and IGF-Ⅰin knee synovial fluid and serum were higher in KOA group compared to control group(BGP:17.91+/-2.51 vs 13.43+/-1.20 μg/l,t=19.735,P=0.000; 7.04+/-2.03 vs 6.08+/-1.96 μg/l,t=9.387,P=0.000; MMP-1:13.79+/-2.51 vs 12.96+/-2.39 μg/l,t=10.979,P=0.000; 8.01+/-2.01 vs 6.79+/-1.57 μg/l,t=17.403,P=0.000; IGF-Ⅰ:15.63+/-3.51 vs 11.63+/-2.87 μg/l,t=40.970,P=0.000; 6.09+/-2.39 vs 4.30+/-2.01 μg/l,t=20.340,P=0.000).Conclusion:Quantitative determination of contents of BGP,MMP-1 and IGF-Ⅰin knee synovial fluid and serum can be used as one of the assisted methods for diagnosis of knee cartilage injuries.
