摘要:
目的:探討中老年女性膝骨關(guān)節(jié)炎(knee osteoarthritis,KOA)患者中醫(yī)體質(zhì)類型特點。方法:選取97例在四川省骨科醫(yī)院就診的中老年女性KOA患者,評定其中醫(yī)體質(zhì)類型、體質(zhì)積分、病程、西安大略和麥克馬斯特大學(xué)(Western Ontario and McMaster Universities,WOMAC)骨關(guān)節(jié)炎指數(shù)及人體質(zhì)量指數(shù)(body mass index,BMI)。分析其中醫(yī)體質(zhì)類型特點及中醫(yī)體質(zhì)類型、體質(zhì)積分與WOMAC骨關(guān)節(jié)炎指數(shù)、BMI的相關(guān)性。結(jié)果:納入研究的97例患者中,平和質(zhì)23例,其余74例均為偏頗體質(zhì),其中陽虛質(zhì)(31例)、血瘀質(zhì)(12例)、氣虛質(zhì)(10例)所占比例較高,陰虛質(zhì)(7例)、氣郁質(zhì)(5例)、痰濕質(zhì)(4例)、特稟質(zhì)(3例)、濕熱質(zhì)(2例)所占比例較小。Pearson相關(guān)分析結(jié)果顯示,陽虛質(zhì)與BMI呈負(fù)相關(guān)(r=-0.298,P=0.001)、氣郁質(zhì)與病程呈正相關(guān)(r=0.150,P=0.041),氣郁質(zhì)患者的體質(zhì)積分與其病程呈正相關(guān)(r=0.714,P=0.042)。結(jié)論:中老年女性KOA患者中醫(yī)體質(zhì)類型以陽虛質(zhì)、血瘀質(zhì)、氣虛質(zhì)等偏頗體質(zhì)為主,臨床治療中應(yīng)注意偏頗體質(zhì)和情志的調(diào)理。
Abstract:
Objective:To explore the feature of type of physical constitution(TCM)in elderly female with knee osteoarthritis(KOA).Methods:Ninty-seven elderly female patients with KOA were recruited from Sichuan Orthopaedic Hospital.The types and scores of physical constitution(TCM),course of disease,Western Ontario and McMaster Universities(WOMAC)osteoarthritis index and body mass index(BMI)were evaluated.The feature of different types of physical constitution(TCM)and the correlation between the,types and scores of physical constitution(TCM)and WOMAC osteoarthritis index and BMI were analyzed.Results:The physical constitution(TCM)of the 97 patients included mild constitution(23)and biased constitution(74),in which yang-deficiency constitution(31),blood-stasis constitution(12)and qi-deficiency constitution(10)took a high proportion,and yin-deficiency constitution(7),qi-depression constitution(5),phlegm-dampness constitution(4),special diathesis constitution(3)and dampness-heat constitution(2)took a small proportion.The results of Pearson correlation analysis showed that yang-deficiency constitution was negatively correlated with BMI(r=-0.298,P=0.001),and qi-depression constitution was positively correlated with course of disease(r=0.150,P=0.041),and constitution scores of patients with qi-depression constitution was positively correlated with course of disease(r=0.714,P=0.042).Conclusion:The types of physical constitution(TCM)of elderly female patients with KOA present mainly with biased constitution such as yang-deficiency constitution,blood-stasis constitution and qi-deficiency constitution,so biased constitution and emotions should be regulated in the clinical treatment.
