摘要:
目的:觀察加味陽和湯對早期膝骨關(guān)節(jié)炎兔關(guān)節(jié)軟骨的影響。方法:將30只6月齡普通級(jí)健康新西蘭大白兔隨機(jī)分為空白組、模型組和實(shí)驗(yàn)組,每組10只。除空白組外,其余2組動(dòng)物均按照改良Hulth法對右側(cè)膝關(guān)節(jié)進(jìn)行早期膝骨關(guān)節(jié)炎造模。造模成功后,實(shí)驗(yàn)組動(dòng)物以加味陽和湯灌胃,模型組動(dòng)物以等量生理鹽水灌胃,連續(xù)4周。藥物干預(yù)結(jié)束后,對膝關(guān)節(jié)進(jìn)行X線檢查和解剖學(xué)大體觀察,并于脛骨平臺(tái)內(nèi)側(cè)取關(guān)節(jié)軟骨制作HE染色切片,光鏡下觀察軟骨改變,按照Mankin評(píng)分標(biāo)準(zhǔn)進(jìn)行評(píng)分。結(jié)果:①X線觀察。空白組兔膝關(guān)節(jié)關(guān)節(jié)間隙對稱,關(guān)節(jié)面光滑平整,無明顯增生; 模型組可見膝關(guān)節(jié)內(nèi)側(cè)間隙基本消失,呈內(nèi)翻畸形,股骨內(nèi)側(cè)髁變大,脛骨內(nèi)側(cè)平臺(tái)不平整,股骨內(nèi)髁及脛骨內(nèi)側(cè)平臺(tái)可見大量增生骨贅; 實(shí)驗(yàn)組可見膝關(guān)節(jié)間隙不平整,內(nèi)側(cè)間隙變窄,程度較模型組輕,股骨內(nèi)髁、脛骨內(nèi)側(cè)平臺(tái)有少量骨贅形成,但較模型組變化小。②大體觀察。按軟骨形態(tài)退化程度由輕至重分級(jí),空白組Ⅰ級(jí)9只、Ⅱ級(jí)1只,模型組Ⅱ級(jí)3只、Ⅲ級(jí)7只,實(shí)驗(yàn)組Ⅱ級(jí)5只、Ⅲ級(jí)5只。3組兔膝關(guān)節(jié)關(guān)節(jié)軟骨形態(tài)比較,空白組最好、實(shí)驗(yàn)組次之、模型組最差(R^-空白組=5.90,R^-模型組=21.35,R^-實(shí)驗(yàn)組=19.25; χ2=20.524,P=0.000)。③鏡下觀察。3組兔關(guān)節(jié)軟骨Mankin評(píng)分分別為(0.550±0.675)分、(7.950±1.535)分、(4.300±1.337)分,組間比較差異有統(tǒng)計(jì)學(xué)意義(F=89.233,P=0.000); 空白組評(píng)分低于模型組和實(shí)驗(yàn)組(P=0.000,P=0.000),模型組高于實(shí)驗(yàn)組(P=0.000)。結(jié)論:加味陽和湯可抑制早期膝骨關(guān)節(jié)炎兔關(guān)節(jié)軟骨的退變,這可能是其治療早期膝骨關(guān)節(jié)炎的作用機(jī)制之一。
Abstract:
Objective:To observe the effect of Jiawei Yanghe Tang(加味陽和湯,JWYHT)on articular cartilage in rabbits with early knee osteoarthritis.Methods:Thirty 6-month-old ordinary healthy New Zealand white rabbits were randomly divided into blank group,model group and experimental group,10 cases in each group.The early knee osteoarthritis models of right knee joint were built by modified Hulth method in rabbits of model group and experimental group.After that,the rabbits in experimental group were intragastric administrated with JWYHT,while rabbits in model group were intragastric administrated with normal saline for consecutive 4 weeks.After drug intervention,X-ray examination and roughly anatomical observation were performed on knee joints.The cartilago articularis were fetched from medial tibial plateau and were sectioned for HE staining.All the samples were observed under the optical microscope and the cartilaginous changes were evaluated according to Mankin Score.Results:Symmetrical knee joint gap and smooth articular surface and no obvious hyperplasia were found in rabbits of blank group.In model group,the medial joint gap almost disappeared and the knee presented with varus deformity; and accrescent medial femoral condyle,rough medial tibial plateau and a large number of osteophytes located at medial femoral condyle and medial tibial plateau were found.In experimental group,the knee joint gap was not smooth and medial knee joint gap became narrow and a few osteophytes were found at medial femoral condyle and medial tibial plateau.In fact,less anatomical changes were found in the experimental group compared to the model group.The cartilage morphology was classified according to its degradation degree,The rabbits belonged to gradeⅠ(9)and gradeⅡ(1)in blank group,gradeⅡ(3)and gradeⅢ(7)in model group,gradeⅡ(5)and gradeⅢ(5)in experimental group.Comparison showed that the blank group was the best,experimental group was good and model group was the worst in knee articular cartilage morphology(R^-blank group=5.90,R^-model group=21.35,R^-experimental group=19.25; χ2=20.524,P=0.000).Mankin scores of cartilago articularis was 0.550+/-0.675,7.950+/-1.535 and 4.300+/-1.337 points respectively,there was statistical difference between the 3 groups(F=89.233,P=0.000).Mankin scores were lower in blank group compared to model group and experimental group(P=0.000, P=0.000),and were higher in model group compared to experimental group(P=0.000).Conclusion:The JWYHT can inhibit the degeneration of cartilago articularis of rabbits with early knee osteoarthritis,so it may be one of the mechanisms of action for treatment of early knee osteoarthritis.
