摘要:
目的:探討掌側(cè)入路與背側(cè)入路對(duì)腕舟骨骨折愈合率的影響。方法:中文數(shù)據(jù)庫以“腕舟骨骨折”與“掌側(cè)入路”和“背側(cè)入路”的布爾邏輯搭配結(jié)果作為檢索詞,英文數(shù)據(jù)庫以“scaphoid fractures”與“dorsal”、“volar”及“palmar”的布爾邏輯搭配結(jié)果作為檢索詞。用計(jì)算機(jī)檢索中國(guó)知網(wǎng)、萬方學(xué)術(shù)期刊數(shù)據(jù)庫、維普中文期刊數(shù)據(jù)庫、中國(guó)生物醫(yī)學(xué)文獻(xiàn)服務(wù)系統(tǒng)、中國(guó)生物醫(yī)學(xué)期刊網(wǎng)引文數(shù)據(jù)庫、Pubmed及Sciencedirect數(shù)據(jù)庫建庫至2013年11月收錄的所有相關(guān)文獻(xiàn)。利用Revman5.2統(tǒng)計(jì)軟件進(jìn)行統(tǒng)計(jì)分析。結(jié)果:共納入9篇文獻(xiàn),涉及435例患者,其中隨機(jī)對(duì)照實(shí)驗(yàn)2篇,非隨機(jī)對(duì)照實(shí)驗(yàn)7篇; 中文文獻(xiàn)4篇,外文文獻(xiàn)5篇。漏斗圖顯示文獻(xiàn)不存在發(fā)表偏倚。掌側(cè)入路組與背側(cè)入路組總的骨折愈合率比較,差異無統(tǒng)計(jì)學(xué)意義[Z=149,P=0.140; OR=0.420,95%CI(0.140,1.310)]; 掌側(cè)入路組術(shù)后6~8個(gè)月的骨折愈合率優(yōu)于背側(cè)入路組[Z=2.840,P=0.004; OR=6.930,95%CI(1.820,26.320)]。掌側(cè)入路組與背側(cè)入路組術(shù)后并發(fā)癥發(fā)生率比較,差異無統(tǒng)計(jì)學(xué)意義[Z=0.590,P=0.560; OR=0.760,95%CI(0.310,1.890)]。掌側(cè)入路組與背側(cè)入路組術(shù)后腕部活動(dòng)功能比較,差異無統(tǒng)計(jì)學(xué)意義[Z=0.990,P=0.320; OR=1.900,95%CI(-1.840,5.840)]。結(jié)論:掌側(cè)入路與背側(cè)入路治療腕舟骨骨折對(duì)骨折愈合率的影響沒有差異,但經(jīng)掌側(cè)入路固定的患者骨折愈合更快。
Abstract:
Objective:To explore the effect of volar approach versus dorsal approach on the healing rate of scaphoid bone fractures.Methods:The Boolean logical combination of Scaphoid fractures,Dorsal,Volar and Palmar were used as index terms.All the literatures of controlled trial included from database establishing to November 2013 were retrieved from China national knowledge internet,Wanfang Database,Vip Database of Chinese journal,Chinese biomedical literature service system(SinoMed),Chinese Medical Citation Index(CMCI),Pubmed and Sciencedirect database through computer,and the statistical analysis was conducted by using Revman5.2 software.Results:Nine literatures(435 patients)were included in the final analysis,including two randomized controlled trials and seven nonrandomized controlled trials,four Chinese literatures and five English literatures.The symmetrical funnel plot demonstrated that there was no publication bias.There was no statistical difference in the total fracture healing rate between volar approach and dorsal approach(Z=149,P=0.140; OR=0.420,95%CI(0.140,1.310)).The volar approach surpassed the dorsal approach in the fracture healing rate 6 to 8 months after the surgery(Z=2.840,P=0.004; OR=6.930,95%CI(1.820,26.320)).There was no statistical difference in postoperative complication incidences between the two approaches(Z=0.590,P=0.560; OR=0.760,95%CI(0.310,1.890)).There was no statistical difference in postoperative wrist activity function between the two approaches(Z=0.990,P=0.320; OR=1.900,95%CI(-1.840,5.840)).Conclusion:There is no difference in fracture healing rate between volar approach and dorsal approach in the treatment of scaphoid bone fracture,while the fracture would heal faster by using volar surgical approach.
