摘要:

目的:比較局部封閉聯(lián)合小針刀和洛芬待因片口服聯(lián)合中藥熱敷治療膝關(guān)節(jié)鵝足滑囊炎的臨床療效。方法:單側(cè)膝關(guān)節(jié)鵝足滑囊炎患者85例,男27例,女58例; 年齡38～81歲,中位數(shù)58歲; 左側(cè)34例,右側(cè)51例; 病程5 d至8年,中位數(shù)11個月。按就診順序隨機(jī)分為2組,觀察組45例,對照組40例; 觀察組采用局部封閉聯(lián)合小針刀治療,對照組采用洛芬待因片口服聯(lián)合中藥熱敷治療。分別在治療前、治療后(觀察組為治療后1周,對照組為治療1周后),采用疼痛視覺模擬評分法對2組患者患膝疼痛情況進(jìn)行評分,并對2組患者的疼痛評分和療效進(jìn)行比較。結(jié)果:治療前2組患者患膝壓痛、自發(fā)痛評分比較,組間差異均無統(tǒng)計(jì)學(xué)意義[(3.166±1.010)分,(3.019±1.069)分,t=0.402,P=0.553;(2.834±0.122)分,(2.791±0.139)分,t=0.523,P=0.326]; 治療后2組患者患膝疼痛均有所緩解,且觀察組疼痛緩解更明顯,壓痛和自發(fā)痛評分均低于對照組[(0.938±0.171)分,(1.990±0.630)分,t=2.256,P=0.012;(0.806±0.220)分,(1.381±0.343)分; t=1.652,P=0.036]; 參照《中醫(yī)病證診斷療效標(biāo)準(zhǔn)》膝部滑囊炎療效評定標(biāo)準(zhǔn)評價療效,觀察組治愈16例、好轉(zhuǎn)24例、未愈5例,對照組治愈8例、好轉(zhuǎn)21例、未愈11例,觀察組療效優(yōu)于對照組(Z=-2.131,P=0.033)。結(jié)論:局部封閉聯(lián)合小針刀治療膝關(guān)節(jié)鵝足滑囊炎,可有效減輕患膝腫脹、緩解疼痛,有利于膝關(guān)節(jié)功能恢復(fù),療效優(yōu)于洛芬待因片口服聯(lián)合中藥熱敷。

Abstract:

Objective:To compare the clinical curative effect of local blocking combined with small knife needle versus oral application of ibuprofen-codeine phosphate tablets combined with hot compress with traditional Chinese drug for treatment of knee articular anserine bursitis.Methods:Eighty-five patients with unilateral knee articular anserine bursitis consisted of 27 males and 58 females,and ranged in age from 38 to 81 years(Median=58 yrs).The anserine bursitis located in left knee for 34 patients and right knee for 51 patients,and ranged in course of disease from 5 days to 8 years(Median=11 months).The patients were randomly divided into 2 groups according to registration order,45 cases in observation group,40 cases in control group.Patients in the observation group were treated with local blocking combined with small knife needle,while patients in the control group were treated with oral application of ibuprofen-codeine phosphate tablets combined with hot compress with traditional Chinese drug.The knee pain degree were evaluated by using visual analogue scale(VAS)before the treatment and after the treatment(observation group:one week after the treatment,control group:after 1-week treatment),and the pain scores and therapeutic effects were compared between the 2 groups.Results:There were no statistical difference in the knee tenderness and spontaneous pain between the 2 groups before the treatment(3.166+/-1.010 vs 3.019+/-1.069 points,t=0.402,P=0.553; 2.834+/-0.122 vs 2.791+/-0.139 points,t=0.523,P=0.326).The knee pain were relieved after the treatment in the 2 groups,and more obvious pain relief were found in the observation group,and the scores of tenderness and spontaneous pain of the observation group were lower than those of the control group(0.938+/-0.171 vs 1.990+/-0.630 points,t=2.256,P=0.012; 0.806+/-0.220 vs 1.381+/-0.343 points,t=1.652,PSixteen patients were cured,24 improved and 5 poor in observation group,while 8 patients were cured,21 improved and 11 poor in control group.The observation group surpassed the control group in the therapeutic effect(Z=-2.131,P=0.033).Conclusion:The therapy of local blocking combined with small knife needle can effectively alleviate pain and swelling of the knee,and it is helpful to the restoration of knee joint function.Its therapeutic effect is better than that of oral application of ibuprofen-codeine phosphate tablets combined with hot compress with traditional Chinese drug in the treatment of knee articular anserine bursitis.Sixteen patients were cured,24 improved and 5 poor in observation group,while 8 patients were cured,21 improved and 11 poor in control group.The observation group surpassed the control group in the therapeutic effect(Z=-2.131,P=0.033).Conclusion:The therapy of local blocking combined with small knife needle can effectively alleviate pain and swelling of the knee,and it is helpful to the restoration of knee joint function.Its therapeutic effect is better than that of oral application of ibuprofen-codeine phosphate tablets combined with hot compress with traditional Chinese drug in the treatment of knee articular anserine bursitis.=0.036).The therapeutic effects were evaluated according to therapeutic effect evaluation standard of knee bursitis which was extracted from Standard for diagnosis and therapeutic effectiveness evaluation of traditional Chinese medicine
