摘要:
目的:觀察補陽還五湯加減方預(yù)防髖關(guān)節(jié)置換術(shù)后深靜脈血栓形成的療效。方法:將收治的77例病人(病例來源為浙江省諸暨市中醫(yī)院住院行髖關(guān)節(jié)置換術(shù)的病人)按隨機對照原則,分為治療組和對照組。手術(shù)為同一組醫(yī)生操作,均采用全麻。術(shù)后治療組給補陽還五湯加減,每天1劑,水煎,早晚分服; 對照組給拜瑞妥10 mg,每天1次口服。給藥2周。觀察治療前后肢體周徑變化、凝血譜變化情況,并做血管彩超檢查觀察血流通暢情況及有無血栓形成。結(jié)果:兩組患者肢體周徑變化(大腿周徑增加值),術(shù)后24 h、3 d、1周、2周組間比較,差異均無統(tǒng)計學(xué)意義(P>0.05); 凝血酶原時間和部分凝血酶活化時間變化術(shù)后3 d、1周、2周比較,差異亦無統(tǒng)計學(xué)意義(P>0.05); 股靜脈血流速度變化,術(shù)后1周與術(shù)前比較,差異有統(tǒng)計學(xué)意義(P<0.05),術(shù)后2周與術(shù)前比較,差異無統(tǒng)計學(xué)意義(P>0.05); 兩組患者術(shù)后下肢深靜脈血栓發(fā)生率比較,差異無統(tǒng)計學(xué)意義(P>0.05)。結(jié)論:補陽還五湯加減方能很好地預(yù)防髖關(guān)節(jié)置換術(shù)后下肢深靜脈血栓形成,其臨床效果與拜瑞妥在統(tǒng)計學(xué)上無顯著差異。
Abstract:
Objective:To observe the effectiveness of Bu Yang Huan Five Decoction in prevention of deep vein thrombosis(DVT)after total hip arthroplasty.Methods:77 patients(patients underwent hip replacement operation in our hospital)operated by the same group of doctors were randomly divided into treatment group and control group,all the patients were operated by the same group of doctors under general anesthesia.Postoperative Patients in treatment group were administrated with Bu Yang Huan Five Decoction modifying soup one dose a day,morning and evening hours service; patiens in the control group took Xarelto 10 mg,once a day orally for 2 weeks.Observe limb circumference,blood coagulation function changes,hemodynamics of vessels by vascular ultrasound and check for the thrombosis before and after treatment.Results:In limb circumference variation,there was no significant difference in statistics(P>0.05)between the two groups of patients with the upper edge of the patella 15 cm thigh circumference increased value,in 24 hours,three days after operation,one week and two phase comparison; there was significant difference in statistics(P<0.05)in femoral vein hemodynamics changes of one week after operation compared with that before operation in both groups,while no difference(P>0.05)appeared between that 2 weeks after operation with that before operation.There was no significant difference in statistics(P>0.05)in the occurrence of DVT after operation between two groups.Conlusion:The Buyang Huanwu Decoction modifying can prevent DVT after THR and it shows no significant difference in clinical effectiveness with the Xarelto.
