摘要:
目的:評(píng)價(jià)改良Sauvé-kapandji術(shù)治療陳舊性下尺橈關(guān)節(jié)脫位的臨床療效和安全性。方法:2008年7月至2009年7月,采用改良Sauvé-kapandji術(shù)治療陳舊性下尺橈關(guān)節(jié)脫位患者7例,男4例,女3例; 年齡16～44歲,中位數(shù)30歲。左側(cè)4例,右側(cè)3例。均有腕部疼痛、腕關(guān)節(jié)活動(dòng)受限等臨床表現(xiàn)。病程10～18個(gè)月,中位數(shù)14個(gè)月。術(shù)后隨訪觀察腕關(guān)節(jié)功能恢復(fù)情況及并發(fā)癥發(fā)生情況。結(jié)果:所有患者均獲隨訪,隨訪時(shí)間10～18個(gè)月,中位數(shù)12個(gè)月。均無(wú)感染和神經(jīng)損傷等并發(fā)癥。參照改良Mayo腕關(guān)節(jié)功能評(píng)分標(biāo)準(zhǔn)評(píng)定療效,優(yōu)5例,良1例,可1例。結(jié)論:改良Sauvé-kapandji術(shù)治療陳舊性下尺橈關(guān)節(jié)脫位,具有手術(shù)操作簡(jiǎn)單、療效好、并發(fā)癥少等優(yōu)點(diǎn); 但是肌腱長(zhǎng)時(shí)間與骨孔摩擦、截骨端與其周?chē)M織相互磨損,均會(huì)損傷腕關(guān)節(jié)周?chē)捻g帶組織,導(dǎo)致腕關(guān)節(jié)不穩(wěn)定,因此其遠(yuǎn)期療效還有待觀察。
