摘要:
目的:觀察塞來昔布(西樂葆)超前鎮(zhèn)痛對全髖關(guān)節(jié)置換術(shù)后的疼痛評分、鎮(zhèn)痛泵的按壓次數(shù)、髖關(guān)節(jié)活動(dòng)度的影響。方法:將46例患者隨機(jī)分為觀察組和對照組,觀察組于術(shù)前1 h給予塞來昔布(西樂葆)400 mg加5 mL溫水口服,對照組術(shù)前禁食禁飲; 兩組術(shù)后均采用芬太尼靜脈自控鎮(zhèn)痛及每12 h給予塞來昔布(西樂葆)200 mg口服。分別于術(shù)后8 h、16 h、24 h、48 h、72 h比較兩組患者疼痛評分、鎮(zhèn)痛泵的按壓次數(shù)及術(shù)后1周、2周的髖關(guān)節(jié)活動(dòng)度。結(jié)果:觀察組術(shù)后最痛程度、72 h內(nèi)平均疼痛程度均小于對照組(P<0.05); 觀察組術(shù)后24 h內(nèi)鎮(zhèn)痛泵的按壓次數(shù)少于對照組(P<0.05); 觀察組術(shù)后1周、2周的髖關(guān)節(jié)活動(dòng)度明顯高于對照組(P<0.05)。結(jié)論:塞來昔布(西樂葆)超前鎮(zhèn)痛能減輕患者全髖關(guān)節(jié)置換術(shù)后的疼痛,同時(shí)減少了鎮(zhèn)痛泵的按壓次數(shù),改善髖關(guān)節(jié)活動(dòng)范圍,有利于增強(qiáng)患者康復(fù)的信心,早期下地負(fù)重,減少臥床并發(fā)癥發(fā)生率,提高生活質(zhì)量。
Abstract:
Objective:To observe the preeptive analgesic effect with celebrex(celeboxib capsules)in the pain score、the press of PCA、the motion of hip in the total hip replacement.Method:46 paients scheduled for the total hip replacement were randomly devided into two groups.The observation group received 400 mg celebrex(celeboxib)with 5ml water 1h before operation,while those of the comparison group didn't.The two groups received PCA and celebrex(celeboxib capsules)per 12 h after operation,to assess the pain score、the press of PCA at the time of 8、16、24、48、72 h after operation and the motion of the hip at 1、2W after operation.Result:the postoperative pain and the average pain in 72 h were lighter than the comparison group(P<0.05),the observation group press of PCA in 24 h of postoperative is less than comparison group(P<0.05),the function of hip in observation group at 1、2W after operation were higher than comparison group(P<0.05).Conclsion:the preeptive analgesic with celebrex(celeboxib capsules)could release the pain of the total hip replacement,and decrease the press of PCA,improve the function of the hip,increase the confidence of paient rehabilitation,immediate full weight-bearing,decrease the rate of complication of bedrid,improve the quality of life.
