摘要:
目的:評價中醫(yī)藥治療骨質(zhì)疏松性椎體壓縮骨折的臨床療效。方法:以“壓縮性骨折”“椎體骨折”“腰椎骨折”“胸椎 骨折”4個關(guān)鍵詞與“中醫(yī)療法”“中藥療法”及“中醫(yī)藥療法”3個關(guān)鍵詞的任意組合為關(guān)鍵詞,用計算機檢索中國知網(wǎng)、萬方 數(shù)據(jù)庫、維普數(shù)據(jù)庫1999年1月至2011年11月期間收錄的所有中醫(yī)藥治療骨質(zhì)疏松性椎體壓縮骨折的隨機對照研究文獻。利用 Cochrane協(xié)作網(wǎng)提供的Revman5.1統(tǒng)計軟件進行統(tǒng)計分析。結(jié)果:共檢索出文獻483篇,通過篩選最終共納入11篇文獻,涉及953例 患者。在骨密度方面,治療組優(yōu)于對照組(Z=2.830,P=0.005),漏斗圖對稱,提示存在發(fā)表偏倚的可能性較小; 在功能改善方面,治 療組顯效率高于對照組(Z=2.580,P=0.010),敏感性分析顯示,前后結(jié)果沒有本質(zhì)改變; 有效率比較,治療組與對照組的差異無統(tǒng) 計學(xué)意義(Z=0.740,P=0.460)。結(jié)論:中醫(yī)藥治療可明顯改善骨質(zhì)疏松性椎體壓縮骨折患者的脊柱功能,提高骨密度,是治療骨質(zhì) 疏松性椎體壓縮骨折的重要方法。
Abstract:
Objective:To evaluate the clinical curative effect of traditional Chinese medicine in the treatment of osteoporotic vertebral compression fractures(OVCF).Methods:All the literatures of randomized controlled trial for traditional Chinese medicine in OVCF treatment included from January 1999 to November 2011 were retrieved from China national knowledge internet,Wanfang Database and Vip Database through computer,and any combination words from the 4 keywords as Compression fracture,Vertebral fracture,Lumbar vertebrae fracture,Thoracic vertebrae fracture and from the 3 keywords as Traditional Chinese medicine(TCM)therapy,Traditional Chinese Drug(TCD)therapy,TCM and TCD therapy were selected as index terms.Statistical analysis was carried on through Revman5.1 software provided by Cochrane collaboration networks.Results:Four hundred and eighty-three literatures in total were retrieved.After selection,11 literatures including 953 patients met the inclusion criteria finally.Bone mineral density(BMD)of treatment group was better than that of control group (Z=2.830,P=0.005),and the symmetrical funnel plot demonstrated that the possibility of existing publication bias was small.On the aspect of the function of knee joint,the excellence rate of treatment group was higher than that of control group(Z=2.580,P=0.010),and the result of sensitivity analysis indicated that there was not essential change in the results before and after the treatment.There was not statistical difference in effective rate between the 2 groups(Z=0.740,P=0.460).Conclusion:Traditional Chinese medicine therapy can obviously improve the spinal function and BMD of OVCF patients,therefore,it is an important method in OVCF treatment.
