摘要:
目的:分析比較股骨遠(yuǎn)端微創(chuàng)內(nèi)固定系統(tǒng)與髁部支持鋼板固定治療股骨遠(yuǎn)端C型骨折的臨床療效。方法:回顧性分析2003年1月至2010年4月在福建醫(yī)科大學(xué)附屬第二醫(yī)院采用股骨遠(yuǎn)端微創(chuàng)內(nèi)固定系統(tǒng)固定治療的63例(Ⅰ組)及采用髁部支持鋼板固定治療的76例(Ⅱ組)股骨遠(yuǎn)端C型骨折患者的病歷資料,比較2組患者的手術(shù)時(shí)間、術(shù)中出血量、骨折愈合時(shí)間及術(shù)后3個(gè)月、6個(gè)月及12個(gè)月時(shí)的HSS膝關(guān)節(jié)評(píng)分。結(jié)果:①Ⅰ組患者的手術(shù)時(shí)間、術(shù)中出血量及骨折臨床愈合時(shí)間均少于Ⅱ組,差異有統(tǒng)計(jì)學(xué)意義(U=3.863,P=0.000; U=3.759,P=0.000; U=5.007,P=0.000)。②兩組HSS膝關(guān)節(jié)評(píng)分比較,存在時(shí)間效應(yīng)(F=105.801,P=0.000),不存在組別效應(yīng)(F=0.344,P=0.558),測(cè)量時(shí)間與處理方案間不存在交互作用(F=0.069,P=0.856)。結(jié)論:對(duì)于股骨遠(yuǎn)端C型骨折患者而言,股骨遠(yuǎn)端微創(chuàng)固定系統(tǒng)和髁部支持鋼板固定相比有術(shù)中出血量少、手術(shù)時(shí)間短、骨折愈合快的優(yōu)勢(shì),但二者對(duì)患者膝關(guān)節(jié)功能的療效無差別。
Abstract:
Objective:To analyse and compare the clinical curative effects between less invasive stabilization system for distal femur(LISS-DF)and condylar buttress plate(CBP)in the type C distal femur fractures.Methods: The medical records of 139 patients with the type C distal femur fractures treated in The Second Affiliated Hospital of Fujian Medical University from January 2003 to April 2010 were analyzed retrospectively.Sixty-three cases in groupⅠwere administrated with LISS-DF,while the others in groupⅡwere administrated with CBP.The operation time,amount of blood loss,the fracture healing time and HSS knee joint scores evaluated at the 3rd,the 6th and the12nd month after the operation were compared between the 2 groups.Results:①The operation time,amount of blood loss and the fracture healing time of patients in groupⅠwere all lower than those of patients in groupⅡ,and there was statistical difference between the 2 groups(U=3.863,P=0.000; U=3.759,P=0.000; U=5.007,P=0.000).②On the aspect of HSS knee joint scores,there was time effect(F=105.801,P=0.000)and there was no group effect(F=0.344,P=0.558).There was no interaction between measurement time and handling method(F=0.069,P=0.856).Conclusion:Compared with CBP,LISS-DF has approximate curative effect on the knee joint functions for the patients with the type C distal femur fractures,but the latter has the advantages of less amount of blood loss,shorter operation time and fracture healing time.
