摘要:
目的:探討兔激素性股骨頭壞死與肝細(xì)胞色素P4503A酶活性的相關(guān)性。方法:將18只SPF級(jí)健康新西蘭家兔隨機(jī)分成3組,即空白對(duì)照組、誘導(dǎo)劑組、抑制劑組,每組6只。空白對(duì)照組以生理鹽水灌胃,誘導(dǎo)劑組肌肉注射苯巴比妥注射液,抑制劑組以伊曲康唑混懸液灌胃。連續(xù)用藥3周后,測(cè)定血清肝細(xì)胞色素P4503A酶活性,并采用肌注甲潑尼龍琥珀酸鈉方法造模。繼續(xù)用藥3周后處死動(dòng)物,制作并觀察病理組織標(biāo)本,計(jì)算股骨頭壞死面積和壞死率。結(jié)果:①肝細(xì)胞色素P4503A酶活性。空白對(duì)照組用藥前后肝細(xì)胞色素P4503A酶活性比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(t=1.859,P=0.122); 誘導(dǎo)劑組用藥后肝細(xì)胞色素P4503A酶活性升高(t=7.482,P=0.002); 抑制劑組用藥后肝細(xì)胞色素P4503A酶活性降低(t=5.060,P=0.004)。用藥前3組肝細(xì)胞色素P4503A酶活性比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(F=1.428,P=0.259); 用藥后3組肝細(xì)胞色素P4503A酶活性比較,差異有統(tǒng)計(jì)學(xué)意義(F=8.577,P=0.001); 兩兩比較,誘導(dǎo)劑組高于空白對(duì)照組和抑制劑組(P=0.018,P=0.003),抑制劑組低于空白對(duì)照組(P=0.045)。②股骨頭壞死面積。3組家兔股骨頭壞死面積比較,差異有統(tǒng)計(jì)學(xué)意義(F=5.289,P=0.005)。組間兩兩比較,誘導(dǎo)劑組壞死面積小于空白對(duì)照組和抑制劑組(P=0.024,P=0.008),抑制劑組大于空白對(duì)照組(P=0.039)。③股骨頭壞死率。3組家兔股骨頭壞死率比較,差異有統(tǒng)計(jì)學(xué)意義(F=4.831,P=0.007)。組間兩兩比較,誘導(dǎo)劑組壞死率小于空白對(duì)照組和抑制劑組(P=0.045,P=0.015),抑制劑組大于空白對(duì)照組(P=0.031)。④肝細(xì)胞色素P4503A酶活性與股骨頭壞死面積及壞死率的相關(guān)性。肝細(xì)胞色素P4503A酶活性與股骨頭壞死面積和壞死率均呈負(fù)相關(guān)(r=-0.740,P=0.001; r=-0.753,P=0.001)。結(jié)論:肝細(xì)胞色素P4503A酶在兔激素性股骨頭壞死中起重要作用,其活性與兔激素性股骨頭壞死的發(fā)生呈顯著負(fù)相關(guān)。
Abstract:
Objective:To explore the correlation between steroid-induced avascular necrosis of femoral head(SANFH)in rabbits and activity of cytochrome P4503A enzymes(CYP3A)in liver cell.Methods:Eighteen healthy SPF New Zealand rabbits were randomly divided into 3 groups as blank control group,inducer group and inhibitor group,6 cases in each group.Rabbits in blank control group were intragastric administrated with physiological saline,rabbits in inducer group were injected into muscle with phenobarbital,while the others in inhibitor group were intragastric administrated with itraconazole suspension.After 3 weeks of continuous administration,the enzyme activity of serum CYP3A was determined and intramuscular injection of methylprednisolone sodium succinate was used to make models.After the next 3 weeks of continuous administration,the rabbits were sacrificed for making pathological tissue specimens which were observed to measure the rates and areas of femoral head necrosis.Results:①The enzyme activity of CYP3A:There was no statistical difference in the rabbits of blank control group before and after the administration(t=1.859,P=0.122).After administration,the enzyme activity of CYP3A of inducer group enhanced(t=7.482,P=0.002),while that of inhibitor group depressed(t=5.060,P=0.004).There was no statistical difference in the enzyme activity of CYP3A among the 3 groups before administration(F=1.428,P=0.259),while there was statistical difference among the 3 groups after administration(F=8.577,P=0.001).The enzyme activity of CYP3A of inducer group was higher than that of blank control group and inhibitor group(P=0.018,P=0.003),and the enzyme activity of CYP3A of inhibitor group was lower than that of blank control group(P=0.045).②The areas of femoral head necrosis:There was statistical difference among the 3 groups(F=5.289,P=0.005).The areas of femoral head necrosis of inducer group were smaller than those of blank control group and inhibitor group(P=0.024,P=0.008),and the areas of femoral head necrosis of inhibitor group were bigger than that of blank control group(P=0.039).③The rate of femoral head necrosis:There was statistical difference among the 3 groups(F=4.831,P=0.007).The rate of inducer group was lower than that of blank control group and inhibitor group(P=0.045,P=0.015),and the rate of inhibitor group was higher than that of blank control group(P=0.031).④The correlations between CYP3A enzyme activity and the areas and rate of femoral head necrosis:There was a negative correlation between CYP3A enzyme activity and the areas and rate of femoral head necrosis(r=-0.740,P=0.001; r=-0.753,P=0.001).Conclusion:CYP3A enzyme in liver cell plays an important role in the steroid-induced avascular necrosis of femoral head in rabbits,and its activity has a significant negative correlation with the occurrence of steroid-induced avascular necrosis of femoral head in rabbits.
