摘要:
目的:從病理學(xué)角度評(píng)價(jià)祛痰逐瘀法預(yù)防家兔激素性股骨頭壞死的療效。方法:按體質(zhì)量均衡原則將30只健康SPF級(jí)新西蘭 家兔隨機(jī)分為4組:正常組(A組)6只,模型組(B組)8只,低劑量組(C組)8只,高劑量組(D組)8只。A、B組予生理鹽水灌胃,10 mL·kg-1; C、D組分別以0.36 g·mL-1和0.72 g·mL-1的自擬祛痰逐瘀方混懸液灌胃,10 mL·kg-1。各組給藥均每天1次,連續(xù)用藥3周。第4周起在繼續(xù)上述處理的同時(shí),在B、C、D組家兔左側(cè)大腿內(nèi)側(cè)肌肉 注射甲潑尼龍琥珀酸鈉,20 mg·kg-1,每天1次,連用3周。藥物干預(yù)結(jié)束后處死各組家兔取股骨制作切片,光學(xué)顯微 鏡下觀察骨髓及骨小梁壞死情況,并應(yīng)用Image-Pro Plus 6.0軟件計(jì)算股骨頭壞死率和壞死面積。結(jié)果:①各組股骨頭壞死面積 比較,差異有統(tǒng)計(jì)學(xué)意義(F=21.104,P=0.000)。組間兩兩比較:C、D組與A組比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.446,P=0.368); A、C、 D組壞死面積均小于B組(P=0.000,P=0.000,P=0.000); C、D組差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.901)。②股骨頭壞死率。各組股骨頭壞死 率比較,差異有統(tǒng)計(jì)學(xué)意義(F=18.029,P=0.000)。組間兩兩比較:C、D組與A組比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.485,P=0.356); A、C 、D組壞死率均小于B組(P=0.000,P=0.000,P=0.000); C、D組差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.828)。結(jié)論:從病理學(xué)角度來(lái)看,應(yīng)用祛痰 逐瘀法可有效預(yù)防家兔激素性股骨頭壞死的發(fā)生。
Abstract:
Objective:To make a pathologic evaluation on the prophylactic effect of the method of expeling phlegm and removing blood stasis on the steroid-induced osteonecrosis of the femoral head(SONFH)in the rabbits.Methods:30 healthy New Zealand rabbits in SPF level were randomly divided into 4 groups according to body mass balance principle,6 cases in normal group(group A),8 cases in model group(group B),8 cases in low dose group(group C)and 8 cases in high dose group(group D).Rabbits in group A and group B were intragastric administrated with physiological saline(10 mL·kg-1 ),and rabbits in group C and group D were intragastric administrated with self-made suspension(10 mL·kg-1)which was made of expeling phlegm and removing blood stasis prescription with the dosage of 0.36 g·mL-1 and 0.72 g·mL-1 respectively. Administration of each group was carried out once a day for consecutive 3 weeks.From the beginning of the 4th week,20 mg·kg-1 methylprednisolone sodium succinate were injected into the internal side muscle of left upper legs of rabbits in group B,group C and group D,once a day for consecutive 3 weeks.After drug intervention,all the rabbits were sacrificed for making slices with their femurs.The necrosis of bone marrow and bone trabeculae were observed under light microscope,then the rates and areas of femoral head necrosis were measured through Image-Pro Plus 6.0 software.Results:①On the aspect of femoral head necrosis areas,there was statistical difference among the 4 groups(F=21.104,P=0.000); there was no statistical difference between group C and group A,between group D and group A(P=0.446,P=0.368); group A,group C and group D were all lower than group B(P=0.000,P=0.000,P=0.000); there was no statistical difference between group C and group D(P=0.901).②On the aspect of femoral head necrosis rates,there was statistical difference among the 4 groups(F=18.029,P=0.000); there was no statistical difference between group C and group A,between group D and group A(P=0.485,P=0.356); group A,group C and group D were all lower than group B (P=0.000,P=0.000,P=0.000); there was no statistical difference between group C and group D (P=0.828).Conclusion:The method of expeling phlegm and removing blood stasis is effective on prevention of steroid-induced osteonecrosis of the femoral head in the rabbits on the standpoint of pathology.
