摘要:
目的:觀察中藥熱奄包外用治療膝骨關(guān)節(jié)炎的臨床療效。方法:將符合要求的135例患者隨機(jī)分為3組,每組45例。Ⅰ組采用中藥熱奄包外用治療; Ⅱ組采用中藥熱奄包結(jié)合雙氯芬酸外敷治療; Ⅲ組單純使用雙氯芬酸外敷治療。參照Lequesne的膝骨關(guān)節(jié)炎嚴(yán)重性和活動(dòng)指數(shù)評(píng)估法制定評(píng)分表,分別于治療前、治療后2周、治療后1個(gè)月及治療后3個(gè)月對(duì)患者進(jìn)行臨床評(píng)分。結(jié)果:治療前后不同時(shí)點(diǎn)之間臨床評(píng)分差異有統(tǒng)計(jì)學(xué)意義(F=374.933,P=0.000)。3種不同治療方法之間,差異也有統(tǒng)計(jì)學(xué)意義(F=8.188,P=0.000)。從各時(shí)點(diǎn)看,Ⅱ組治療后2周、治療后1個(gè)月的評(píng)分低于Ⅰ組和Ⅲ組(治療后2周:P=0.000,P=0.000; 治療后1個(gè)月:P=0.006,P=0.000),治療后3個(gè)月Ⅰ、Ⅱ組評(píng)分均低于Ⅲ組(P=0.014,P=0.000); 各時(shí)點(diǎn)其余組間比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義。治療方法與時(shí)間之間存在交互效應(yīng)(F=4.357,P=0.001)。結(jié)論:中藥熱奄包治療膝骨關(guān)節(jié)炎短期療效不及中藥熱奄包加雙氯芬酸外用聯(lián)合治療,但也能從根本上改善本病的病理改變,中期療效與其相當(dāng)。
Abstract:
Objective:To observe the clinical curative effect of the external application of traditional Chinese medicine(TCM)REYANBAO on knee osteoarthritis.Methods:135 patients met the criteria were randomly divided into 3 groups,45 cases in each group.Patients in groupⅠwere administrated with the external application of TCM REYANBAO,and patients in groupⅡwere administrated with the external application of TCM REYANBAO combined with diclofenac,while the others in groupⅢwere administrated with the external application of diclofenac only.A score sheet was drawn up with reference to Lequesne knee osteoarthritis severity and activity index evaluation method,and the clinical effects were scored according to above sheet before the treatment,2 weeks,1 month and 3 months after the treatment respectively.Results:There was statistical difference in clinical scores among different time points(F=374.933,P=0.000).There was statistical difference among the 3 different treatment methods(F=8.188,P=0.000).From the view of each time point:2 weeks and 1 month after the treatment,the scores of groupⅡwere lower than those of groupⅠand groupⅢ(2 weeks after treatment:P=0.000,P=0.000; 1 month after treatment:P=0.006,P=0.000); 3 months after the treatment,the scores of groupⅠand groupⅡwere both lower than those of groupⅢ(P=0.014,P=0.000); there was no statistical difference in scores between other couple groups.There was interaction effects between treatment method and times(F=4.357,P=0.001).Conclusion:Although the short-term effects of TCM REYANBAO is not as good as that of the external application of TCM REYANBAO combined with diclofenac in the treatment of knee osteoarthritis,it can improve the pathological changes of this disease fundamentally and it has the similar intermediate-term effects to that of the above combination method.
