摘要:
目的:觀察復(fù)陽(yáng)活骨丸配合髓芯減壓術(shù)對(duì)激素性股骨頭缺血壞死家兔模型股骨頭骨內(nèi)壓和生物力學(xué)性能的影響,探討其防治股骨頭缺血性壞死的可能機(jī)制,為該方法的臨床應(yīng)用提供依據(jù)。方法:將45只健康家兔隨機(jī)分為正常對(duì)照組(A組)、模型對(duì)照組(B組)、復(fù)陽(yáng)活骨丸組(C組)、髓芯減壓組(D組)、復(fù)陽(yáng)活骨丸+髓芯減壓組(E組)。除A組外,其余各組家兔建立激素性股骨頭壞死模型。造模成功后,A、B兩組不作治療,C組采用復(fù)陽(yáng)活骨丸灌胃治療,D組行髓芯減壓術(shù)治療,E組采用復(fù)陽(yáng)活骨丸灌胃加髓芯減壓術(shù)治療。治療6周后測(cè)定各組家兔股骨頭骨內(nèi)壓,并檢測(cè)股骨頭生物力學(xué)參數(shù)。結(jié)果:各組股骨頭骨內(nèi)壓比較,差異有統(tǒng)計(jì)學(xué)意義(F=7.894,P=0.000); 組間兩兩比較,B、C組均高于A組(LSD-t=10.284,P=0.000; LSD-t=8.356,P=0.000),D、E組與A組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(LSD-t=0.574,P=0.683; LSD-t=0.654,P=0.725); C、D、E組均低于B組(LSD-t=-4.232,P=0.015; LSD-t=-5.241,P=0.000; LSD-t=-6.766,P=0.000); C組高于D、E組(LSD-t=3.876,P=0.027; LSD-t=4.031,P=0.013); D組與E組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(LSD-t=0.562,P=0.939)。生物力學(xué)指標(biāo)測(cè)定,B組骨強(qiáng)度、骨剛度、彈性模量、破裂強(qiáng)度等均呈現(xiàn)骨質(zhì)疏松表現(xiàn),C、D、E組上述指標(biāo)均有所改善,且E組改善優(yōu)于C、D兩組。結(jié)論:復(fù)陽(yáng)活骨丸配合髓芯減壓術(shù)可有效治療激素性股骨頭缺血壞死。其可能機(jī)制為改善股骨頭內(nèi)微循環(huán),扭轉(zhuǎn)股骨頭缺血狀態(tài),促進(jìn)骨小梁修復(fù),降低骨內(nèi)壓,提高骨強(qiáng)度,阻止股骨頭變形,從而達(dá)到治療股骨頭缺血性壞死的目的。
Abstract:
Objective:To observe the effect of FUYANG HUOGU(復(fù)陽(yáng)活骨)pill combined with core decompression on the intraosseous pressure within the femoral head and biomechanical parameters for the rabbit model with steroid induced avascular necrosis of femoral head(SANFH).Methods:45 healthy rabbits were randomly divided into normal control group(group A),model control group(group B),FUYANG HUOGU(復(fù)陽(yáng)活骨)pill group(group C),core decompression group(group D)and FUYANG HUOGU(復(fù)陽(yáng)活骨)pill combined with core decompression group(group E).SANFH rabbits models were built for all the groups except for group A.After successful modeling,rabbits in group A and group B were administrated with no treatment,rabbits in group C were intragastric administrated with FUYANG HUOGU(復(fù)陽(yáng)活骨)pills,rabbits in group D were administrated with core decompression,and rabbits in group E were administrated with FUYANG HUOGU(復(fù)陽(yáng)活骨)pill combined with core decompression.6 weeks after treatment,intraosseous pressure within the femoral head and the biomechanical parameters of the femoral head of rabbits in the 5 groups were evaluated.Results:On the aspect of intraosseous pressure within the femoral head of the rabbits,there was statistical difference among the 5 groups(F=7.894,P=0.000); intraosseous pressure within the femoral head of group B and group C was higher than that of group A respectively(LSD-t=10.284,P=0.000; LSD-t=8.356,P=0.000); there was no statistical difference between group D and group A,between group E and group A(LSD-t=0.574,P=0.683; LSD-t=0.654,P=0.725); intraosseous pressure within the femoral head of group C,group D and group E was lower than that of group B respectively(LSD-t=-4.232,P=0.015; LSD-t=-5.241,P=0.000; LSD-t=-6.766,P=0.000); ntraosseous pressure within the femoral head of group C was higher than that of group D and group E respectively(LSD-t=3.876,P=0.027; LSD-t=4.031,P=0.013); there was no statistical difference between group D and group E(LSD-t=0.562,P=0.939).Bone strength,bone rigidity,elastic modulus and rupture strength of group B were all reflected as osteoporosis,while these biomechanical parameters were all improved in group C,group D and group E,and these biomechanical parameters of group E were improved more obviously than those of group C and group D.Conclusion:The method of FUYANG HUOGU(復(fù)陽(yáng)活骨)pill combined with core decompression performs well on the treatment of SANFH through improving the microcirculation within the femoral head,reversing the state of avascular necrosis of the femoral head,promoting the repair of trabecular bone,reducing intraosseous pressure,improving bone strength and preventing from the femoral head deformation.
