摘要:

目的:比較旋后牽伸手法與傳統(tǒng)內(nèi)旋伸肘頓拉手法治療肱骨外上髁炎的臨床療效。方法:將符合標(biāo)準(zhǔn)的126例肱骨外上髁炎 患者隨機(jī)分為2組。Ⅰ組64例,采用旋后牽伸手法治療; Ⅱ組62例,采用內(nèi)旋伸肘頓拉手法治療。治療1個(gè)療程后參照Verhaar網(wǎng)球 肘療效評(píng)分標(biāo)準(zhǔn),從疼痛、壓痛、握力、關(guān)節(jié)功能、患者滿意度6個(gè)方面對(duì)患者的療效進(jìn)行評(píng)價(jià)。同時(shí)嚴(yán)格記錄患者在治療期間 出現(xiàn)的各種不良反應(yīng)。結(jié)果:①治療后兩組患者疼痛、壓痛、握力、關(guān)節(jié)功能、患者滿意度均較治療前改善,且Ⅰ組改善情況優(yōu) 于Ⅱ組(P<0.01); ②Ⅰ組總體療效優(yōu)于Ⅱ組,差異有統(tǒng)計(jì)學(xué)意義(Z=5.869,P=0.000); ③Ⅱ組8例患者有一過(guò)性的輕度疼痛加 重,經(jīng)1～3 d休息或理療后消失,其余患者未出現(xiàn)任何不良反應(yīng)。結(jié)論:采用旋后牽伸手法治療肱骨外上髁炎,療效優(yōu)于內(nèi)旋伸肘 頓拉手法。

Abstract:

Objective:To compare the therapeutic effect of the internal rotation stretching and paroxysmal pulling manipulation with that of traction manipulation in supination position on external humeral epicondylitis.Methods:126 patients were randomly divided into 2 groups with 64 cases in GroupⅠand 62 cases in GroupⅡ.Patients in GroupⅠwere treated with traction manipulation in supination position while those in Group Ⅱwere treated with internal rotation stretching and paroxysmal pulling manipulation.The indexes involving pain,tenderness,the grip strength of the affected hand,the function of the arthrosis and the satisfaction degree of the patients were documented after the 20-day course of treatment.Verhaar cure effect appraisal system of tennis-ball elbow were used to evaluate the therapeutic effects.Adverse reactions were documented during the 20-day course of treatment.Results:The indexes improved after the treatment in two groups and the improvements in GroupⅠwere superior to that in GroupⅡ(P<0.01).The general therapeutic effects in GroupⅠwere superior to that in GroupⅡ(Z=5.869,P=0.000).8 patients in GroupⅡunderwent temporary aggravation of pain and the symptoms resolved after rest for 1 to 3 days or physical therapy,no adverse reactions occurred in other patients.Conclusion:The therapeutic effect of traction manipulation in supination position is better than that of internal rotation stretching and paroxysmal pulling manipulation in the treatment of external humeral epicondylitis.
