摘要:
目的:比較3種閉合復(fù)位內(nèi)固定法治療新鮮肩鎖關(guān)節(jié)全脫位的療效。方法:136例新鮮肩鎖關(guān)節(jié)全脫位患者,閉合復(fù)位后采用 經(jīng)皮“肩峰-鎖骨”雙枚鋼針固定治療31例(A組),采用經(jīng)皮“肩峰-鎖骨”雙枚鋼針固定+經(jīng)皮縫合肩鎖關(guān)節(jié)囊治療45例(B組),采 用經(jīng)皮“肩峰-鎖骨”雙枚鋼針固定+經(jīng)皮“鎖骨-喙突” 空心螺釘固定+經(jīng)皮縫合肩鎖關(guān)節(jié)囊治療60例(C組,“三聯(lián)固定”組), 參照Karlsson評(píng)價(jià)標(biāo)準(zhǔn)比較3組療效。結(jié)果:3組療效之間的差異有統(tǒng)計(jì)學(xué)意義(χ2=21.623,P=0.002),C組療效優(yōu)于B 組(u=2.014,P=0.002),B組療效優(yōu)于A組(u=2.781,P=0.006)。結(jié)論:經(jīng)皮“肩峰-鎖骨”雙枚鋼針固定+經(jīng)皮“鎖骨-喙突” 空心 螺釘固定+經(jīng)皮縫合肩鎖關(guān)節(jié)囊治療新鮮肩鎖關(guān)節(jié)全脫位,療效優(yōu)于經(jīng)皮“肩峰-鎖骨”鋼針固定+經(jīng)皮縫合肩鎖關(guān)節(jié)囊,更優(yōu)于單 純“肩峰-鎖骨”鋼針固定。
Abstract:
Objective:To compare the curative effect of 3 different types of closed reduction and internal fixation on fresh complete acromioclavicular dislocation.Methods:136 patients sustained complete acromioclavicular dislocation were divided into 3 groups.31 cases in Group A were treated with acromioclavicular closed reduction and acromioclavicular internal fixation by 2 steel-wire per cutem.45 cases in Group B were treated with acromioclavicular closed reduction and acromioclavicular internal fixation by 2 steel-wire per cutem and suturation of the capsula articularis acromioclavicularis per cutem.60 cases in Group C were treated with acromioclavicular closed reduction and acromioclavicular internal fixation by 2 steel-wire per cutem and suturation of the capsula articularis acromioclavicularis per cutem and fixation with a hollowed screw between the clavicle and the coracoid(three point fixation).The curative effects were evaluated according to Karlsson's standard.Results:There was statistical difference in the curative effects between the 3 groups(χ 2=21.623,P=0.002).Curative effects in Group C were superior to those in Group B(u=2.014,P=0.002)and Group B outstriped Group A(u=2.781,P=0.006).Conclusion:The “three point fixation” is a good method for acromioclavicular dislocation and should be popularized with its advantages of reliable fixation and good curative effect.
